On 11-11-2015, [redacted] presented his pet, [redacted] , a 12.5#, 3 year old, female Dachshund for an exam and consultation for anxiety. This was their first visit into our hospital. As regulated by state law, an exam was performed on [redacted] to establish a valid... veterinary-client-patient relationship. Diagnostic blood work was recommended to [redacted] to ensure the pet had no underlying disease mechanism present that would preclude the use of behavior modifying drugs. The owner was given an estimate for the cost of this and approved. Bloodwork was normal for his pet establishing no underlying diseases. The owner requested a written prescription. [redacted] was given a written prescription as he requested. The prescription was written for Clomicalm 20mg (30ct) with the following prescription labeling: “Give ½ tablet by mouth once daily for behavior modification. If after 30 days, results are not seen, increase dose to 1 tablet by mouth once daily.” Clomicalm, made by Novartis a division of Elanco Animal Health, is a prescription medication FDA approved for veterinary use in dogs. Clomicalm is available as 20mg, 40mg and 80mg tablet strengths. The usual, recommended dose of Clomicalm for dogs is 0.9 to 1.6 mg per pound every 24 hours. Thus, [redacted] ’s calculated dosage range is 11.25mg to 20mg every 24 hours. Clomicalm is a tricyclic antidepressant and affects chemicals in the brain (serotonin) that may become unbalanced leading to behavioral problems in dogs. Clomicalm Tablets are used for the treatment of anxiety in dogs. After leaving our office, [redacted] called and stated that he could not get the Clomicalm filled in anything other than a 25mg capsule and needed the prescription re-written. His phone call was returned and [redacted] was told that the prescription could not be filled in a 25mg capsule as the pet only needed 10mg in the initial 30 days and 20mg as a maximum dose and a 25 mg capsule could not be safely cut in 1/2. He was told that he could fill the current written prescription through any pet pharmacy online or any veterinary pharmacy, including, but not limited to, our office. The FDA, the Texas Veterinary Medical Association and the American Veterinary Medical Association all advise that when a pet approved prescription is available, that the veterinarian should prescribe the pet approved form of medication. Clomicalm, made by Novartis, is the pet approved form of this medication. Mr. McIntosh’s complaint should be directed to the pharmacies falsely claiming they carry and can fill pet prescriptions when they clearly do not and for wrongfully advising substitution of both an incorrect dose for his dog and a human approved form of the drug. Prescribing the animal approved form of this drug at the proper dosing strength was in the best interest of [redacted] 's pet, [redacted] . [redacted] was not refused a written prescription. He was advised on the proper form of this medication through our office. Where and with whom he chooses to fill this medication is at his discretion.
