Specialists by her primary care physician, Dr. [redacted]. Ms. [redacted]'s visit was seen by Dr. [redacted] on September 16, 2014. At the time of Ms. [redacted]'s visit, she advised her doctor of an injury which occurred on September 10, 2014 or September 12, 2014.Summit Orthopedic received the referral Dr. [redacted]'s office on September 16, 2014. Summit Orthopedic received authorization for the MRI from the insurance company on October 2, 2014.

Summit Orthopedic contacted Ms. [redacted]'s father on October 6, 2014 to schedule an appointment to discuss the results of the MRI.
Ms. [redacted] was seen in follow-up on October 16, 2014 MRI, the diagnosis and prognosis was discussed in detail with Ms. [redacted] was instructed to brace her knee as needed for pain. Modifications of activity were also discussed. Physical therapy was prescribed and a written prescription was provided to the patient. Ms. [redacted] was to return to Summit Orthopedic in 1 month.
A follow-up appointment was scheduled for November 17, 2014. The patient's father called to reschedule the appointment. It was rescheduled to December 4, 2014.
The patient was a no-show for her December 4, 2014 appointment.
On December 30, 2014 Summit Orthopedic received a physical therapy report indicating the patient has been discharged.
Throughout Ms. [redacted]'s care and treatment with Summit Orthopedic Specialists, Mr. [redacted] refused to pay any of the co-payments for his daughter's treatment. Summit Orthopedic billed Mr. [redacted] and to date no payment have been received.
Summit Orthopedic has a balance due and owing from Mr. [redacted].

In response to Mr [redacted] or ([redacted]) on Behalf of Dr. [redacted] and Summit Orthopedics.  I will address each issue to be as clear as I can.
The first 3 paragraphs are leading up to the visit. No need to address. However the 3rd paragraph as stated “the patient my daughter was assigned to Dr.[redacted]e” not an assistant for evaluation as stated in paragraph 4.
The facts of paragraph 5 are incorrect. My daughter was first seen by a woman (not sure of the name) who came in and started to look at her ankle and address her issues with an ankle! First fact. Then after I addressed the issue that she has not an ankle injury it’s her knee, she realized it was the wrong patient.
Paragraph 6,7, and 8 are correct with the exception to your statement in paragraph 8 that Summit Orthopedic contacted me, that is false, I reached out to them and they were not sure and would call me back. After we could not get back on the schedule until the 16th, 2014, I called everyday based on the advertisement of same day appointments and still could not get her in……..
Paragraph 9, still no Dr. [redacted]e only an assistant.
Paragraph 10, False facts,  Dr. [redacted]e’s office called to reschedule the appointment since he would be out of town for the holidays and the soonest they could get her in would be December 4th.
Paragraph 11, Fact: We showed up but they gave us the wrong time! And asked us to come back at 1:30pm as we waited until 3:30pm and we still did not get called back to be checked by the Doctor.
Paragraph 12,  not sure when they received for counsel if retained will look into.
Paragraph 13 , I offered each time to pay as there is a co-pay, your office did not know what amount was due and said they would send a bill. I have it as its for $30.  Do the math, paragraph 5 states you seen us as well as in 9 and 11. That total is $45 in co-pays, as this goes to show you how off your accounting is as well and your facts in this issue.
Paragraph 14, Summit has a balance due to compensate for Dr. [redacted]e’s ignorance of his office and my time.
Paragraph 15, False to cover your client’s reputation that all you have to do is google the office and you will find similar reviews. My daughter DID NOT receive prompt or adequate care from Summit Orthopedic. Fact, Dr [redacted]e had to hire outside representation to cover his company’s inability to keep up with demand or need for an orthopedic surgeon. Summit should not falsely advertise same day appointments, when I, for several weeks daily reached out for my daughter to be seen to have direction on treatment based on her injury only to have them in our first contact that they send the wrong Dr for the wrong injury with the wrong patient! I have no problem calling it as I see it. I hope that NO ONE else receives the same type of attention as my 14 year old daughter has received from Summit Orthopedics.

Review: This letter is to inform you that I [redacted] (Father of [redacted]) is requesting a refund for all serves performed by SUMMIT Orthopedic specialist as well as compensation for my time.

1) My daughter was referred to your office and to Dr [redacted] for a knee injury. It took over 6 weeks before an appointment could be made. I contacted your office daily to see if there would be a cancelation to get her in for a review and direction for treatment. (as she was in paint and swollen) You advertise same day appointments and during this time you had nothing available, as I was willing to travel to you other office over an hour away if needed offered by the phone attendant, and still no appointment available.

2) My daughter and I kept our original appointment and scheduled only to be in the waiting room approximately 70 minutes, and then we were taken to the Dr Office and waited antoher 20 minutes to be only seen by the WRONG Dr who thought my daughter had an ankle injury from soccer...... This did not give me reassurance that SUMMIT is a very good place. I stayed as it took this long to make an appointment happen so after spending 3 hours dealing with an unorganized and unprofessional staff, My daughter was finally seen by an assistant, to whom recommended us an MRI.

3) I had contacted your office manager and she reassured me this is not normal and they would do better next time. This was not the case....We returned for the a checkup after going to physical therapy only to be told our appointment was a different time that what had originally been scheduled. So they rescheduled us for that afternoon at 1:30 and we returned only to wait until 3:30 in the waiting room and I approached the desk and said we were leaving since its obvious you have double book and this office was way to unprofessional and the receptionist stated to me that our room has been ready for 30 minutes she was not sure why we have not been seen yet.

To sum up this experience, you as a medical professional are overbooking and creating issues that could lead to malpractice suite. I would hope you can take a look from the outside and creating issues that could lead to a malpractice suite. I would hope you can take a look from the outside in and see it from a client's perspective. At no time did my daughter be seen by Dr [redacted] or a qualified Dr for her injury.

SUMMIT? What does this stand for? You have it on your wall in the waiting room yet Dr [redacted] you and your staff have a long way to go before you can ever can claim that you have reached the highest level of quality care one could ask.Desired Settlement: 1) I'm officially requesting my time to be compensated, collectively in dealing with your unprofessional office. I calculate the amount of time between phone calls as well as wait time with time off of work in the amount of 20 hours. My rate of pay is 200 per hour when away from my business in a total amount of $4,000.00.

2) A refund to my insurance company for any and all services you claimed you have performed.

3) An apology from Dr [redacted] for never seeing my child once in all the time she has been striving to make herself stronger so that there knee becomes healthy coming into adult hood.
