[redacted] Ave. We never received the original request, as it was not sent to [redacted], PC's address. Please send further correspondence to Summit [redacted] Box XXXXX, Fort Wayne, IN XXXXX.
Said patient did call and ask for billing records. According to our records these were sent to the patient on 07/07/2017 along with the signed consent he signed at the hospital he was treated at.
If patient has not received the information he requested, Summit Radiology, PC will be more than happy to send them to the supplied address again. If they are sent again they will be sent certified.

Per the Fair Debt Collection Practices Act, I have 30 days from the day I RECEIVE the documentaion to reapond. I am currently reviewing the documentation. I can say for certain that they did not send me what I asked for and a follow up letter will be sent upon my complete review of the documents they did manage to send.
