This is Dr [redacted] ***The first contact I had with the complaintant was in a free roughly half hour phone interviewAt this time she went over her health complaints and we started talking about the blood tests that in my medical opinion would help elucidate what was going on for herWe next met in a ninety minute in person office visitAt this time I went over the physiology of her complaints and how the lab tests worked and what they would tell usI offered to let her decide if she wanted to do the lab tests later or run them at that visitShe chose to do a blood draw in the office and left happy and confident in our treatment planThe first I knew that she was upset was when the lab called and said she had tried to cancel the testsI promptly left her a voice mail message asking her what was going onShe called again and left a message saying she had sent me an emailI called back telling her I had not received an email and that in order to formally cancel the lab tests (they were on hold at this time) I needed to speak to her in person to verify that it is what she wanted and that somebody else wasn't trying to cancel them for herThat is the final communication between usShe has not called back and I have not received any emails from her, nor has she come into the officeI am happy to refund her moneyHowever I don't keep credit card information and I need to verify an address before I can send a check

This complaint is filed against [redacted] ***, a practitioner that simply rents space in my office and is not part of my corporation which is [redacted] or [redacted] Her name is [redacted] and her business name is Sunny Day Health Please remove this complaint off my record This was the first notice I received of this

[redacted] contacted me via email today to say she would be happy to refund my money, which is the first email correspondence I have received in this regardAs per her request, I called the office this afternoon to make sure they have my correct address to send a checkI will notify Revdex.com when I receive a check and this matter will be satisfactorily resolved

(The consumer indicated he/she DID NOT accept the response from the business.) Still awaiting refundCalled office to give them my address on December 21, as requestedI assume they are just delayed by the holidays

My complaint was filed against the wrong business nameI filed my complaint against *** *** and Sunny Day Health, but Revdex.com filed against *** ***, who shares the office space
*** *** called me yesterday to request the complaint be refiled under the correct nameCan you please do this? Thank you

We next met in a ninety minute in person office visit. At this time I went over the physiology of her complaints and how the lab tests worked and what they would tell us. I offered to let her decide if she wanted to do the lab tests later or run them at that visit. She chose to do a blood draw in the office and left happy and confident in our treatment plan.
