We assumed care of this patient on 12/20/2013- that was for a new patient exam- treatment was diagnosed, discussed with the patient, treatment plan/financial responsibility signedPatients next visit was 7/16/2014- recall exam with cleaning no x-raysDr [redacted] again recommend fillings for the upper right decay- Patients next visit was 1/30/for a recall exam with cleaning and x-rays- Dr [redacted] still recommended decay needing to be restored2/19/was a restorative appointment for #2,3,5,14,19,20,28,30,all restored with compositePatients next visit was 3/12/Emergency exam for tooth #fractured (previous root canal) restored with MILF composite, Complete exam with x-rays and cleaning also done that same visitNext visit was 3/24/for restorative fillings #10,11,20,21,28,29,Patients next visit was for Crown #8- previous root canal- Treatment plan/financial plan signedCrown seat #4/28/The patient asked if she could make monthly payments until the account was paid off for DOS 04/28/this agreement was made on 05/08/Then payments allowed monthly were different amounts each month to help with the patients financial situationShe was seen on 08/31/for an emergency exam for tooth number We performed the RCT of #along with an exam and an X-Ray of that toothPost op call was done on 09/02/to see how the patient was doing a message was left no call back notedPayments continued as prior agreed upon until 12/29/Patient called she had gotten a new job and requested that January's payment was postponedThe last payment we received was on 02/23/of leaving her balance at And we have no received payment sinceFinal notice sent 10/1/for the account to be paid in fullWe are sending the complete chart and account notes from 12/20/to 10/25/Along with all of the X-Rays taken along with every single signed treatment plan/financial agreement for said dates of serviceWe are sending it to the [email protected] along with the [email protected] believe this complaint is financially motivated due to the statements the patient received and the final notice for payment, when we've been more than accommodating for her situation

Patients next visit was 3/12/2015 Emergency exam for tooth #8 fractured (previous root canal) restored with MILF composite, Complete exam with x-rays and cleaning also done that same visit. Next visit was 3/24/2015 for restorative fillings #10,11,20,21,28,29,30. Patients next visit was for Crown #8- previous root canal- Treatment plan/financial plan signed. Crown seat #8 4/28/2015. The patient asked if she could make monthly payments until the account was paid off for DOS 04/28/15 this agreement was made on 05/08/15. Then payments allowed monthly were different amounts each month to help with the patients financial situation. She was seen on 08/31/15 for an emergency exam for tooth number 19. We performed the RCT of #19 along with an exam and an X-Ray of that tooth. Post op call was done on 09/02/15 to see how the patient was doing a message was left no call back noted. Payments continued as prior agreed upon until 12/29/15. Patient called she had gotten a new job and requested that January's payment was postponed. The last payment we received was on 02/23/16 of 200.00 leaving her balance at 640.50. And we have no received payment since. Final notice sent 10/1/16 for the account to be paid in full. We are sending the complete chart and account notes from 12/20/2013 to 10/25/16. Along with all of the X-Rays taken along with every single signed treatment plan/financial agreement for said dates of service. We are sending it to the [email protected] along with the [email protected]. We believe this complaint is financially motivated due to the statements the patient received and the final notice for payment, when we've been more than accommodating for her situation.
