On 11/29/18, I used their transportation services While on the van, my wheelchair flipped backwards, causing me to sustain injuries to my head, back, neck shoulders, pelvic area and chest My wheelchair was completely damaged I went to the ER and was treated right afterwards My in incident was caused because the driver did not properly secure my wheelchair She did not have the front locks on the chair or a seatbelt on me I contacted the owner or the company the same day and he assured me that what ever info I needed would be available to me and that he was about to contact the insurance company and he would call me back, which he didn't I called him back on 12/3/and explain to him that I needed my wheelchair replaced because that was the way I am transportedHe told me that he was going to talk with his attorney and he would call me back He didn't return the call so, I called him back on 12/4/and asked about the chair he then told me that he and his attorney had
