We saw Mrs*** on 11-8-13and agreed to do aligners.The cost is 5,and I gave her a discount just charge her 4,000.Then she called and wants to wait for for she found out that her insurance will cover her in although the
aligners were ready in December.We made an appointment for her 1-21-so she can use her 1,orthodontic coverage.We take an upper and lower molds of teeth regardless if you are doing one or two appliances so the technician will have a study model on the occlusion/bite of the patientAnd this is done by all dentist when you are ordering any type of appliance.I told her I will see once I study your case if she needs upper and lower aligners.After I studied her case and consulted the technician, the technician wants to fabricate the lower only and use the upper as a guide and see if it can be done in aligners.I can always order additional aligners if necessary.You will have the final passive(non-active) retainer/aligner on the upper when the case is finish.You are not paying the amount of aligners, you are paying to fix the problemAs you go along with the appointment extra impression of the mouth is taken as a follow up on how the treatment is progressing.If the 3rd aligners fixes the problem a permanent upper retainer/aligner will be orderedIf not ready the dentist re-evaluate the case.This type of aligners depends on highly on patients cooperation.They can be finish early if the patient is wearing them as the doctor recommended and takes longer if the patient does not comply.If she does not comply or does not like the aligners other orthodontic treatment like the regular brackets can be installed but there will be additonal charges.The bracketed one is the last resort ,for these are glued to your teeth and they work day and night for the patient cannot remove them unlike the aligners you can take them off anytime.This is only is she does not like the aligners.We did not even have the chance to see her for her first adjustment.She said we did not call her.It was her instruction to us to install the aligners in January for insurance purposes.She mentioned that she has a time frame since they are military familyHer consultation to the other dentist was just a general response to an inquiry on how long is the ideal time for aligmers, months.It can always be shorter since according to her herself ,her teeth are not really bad only the lower was slightly crooked.If she went to another dentist after the dentist takes the mold and send them to the dental laboratory, the technician will talk and discuss the case with the dentist on how many aligner this case needsThe length of time will depend on patient complianceShe was given an option to go with my treatment plan or go with the other dentist treatment planYou cannot have them both for every dentist have their own techniques and opinionWe offered her to have her teeth cleanedShe said she don't want it, so we did not force herYou don't have to wait months to have your teeth cleaned and in her case since we are installing an appliance it was a good idea to have her teeth cleaned if she wants toWe did not charge her or send any claims to her insurance, so no refund necessaryShe can return the aligners and we will refund her money

I will ACCEPT the offer AFTER reviewing the new one page release form
Military Navy Legal office and it's legal team has been more than helpful during this process and was able to clear up the FACT that two sheets were mailed to me and that now there is an "updated" compressed release form which only contains one page
Additional comments:
-the business and its employees can lie but I am more than willing to take a polygraph test to prove:
- I have called during business hours and left voice message (fyi - Navy legal said my phone records would hold up in court if need be)
- I have spoken with ***
- I did speak up and say something at the time of receipt that I thought I was getting top and bottom aligners
- you sent me an incomplete release form
Again please do NOT try and threaten me about going to court b/c like I have said over and over again I have NOTHING to hide and have no problem taking a polygraph test proving I called numerous times during business hours, left voice message, left a message with Sheena and also the fact that you sent me and incomplete documentWe can all take the test and prove who the real liars are here!!
The public needs to be made aware of businesses like this that lie!
