communication we received from patient was on September 14, 2015 where patient called and stated he would like to make three equal payments on account and will call in with first payment soon.  Final statement sent on 9/29/15 states "Your account is scheduled to go to our collection agency.  Please remit payment within five days to prevent this action.  Reminder - payments can now be made online at [redacted]".  No payment had been received by October 15, 2015 therefore patient's account was sent to collections.
