At first, in her file it said that the consumer is to be self medicated, but they provided no reminder/check in to make sure that she has the medication she needs/has been taking it. They have not been bringing along with them her paperwork that breaks down her medication to even be able to legally assist her with medication concerns.

The staff have left her alone multiple times, despite being a 24 hr case and have been verbally abusive when she addresses her concerns. They have not been bringing paperwork, like the breakdown of services provided. The only paperwork in her book is her ISP, they do not have paperwork about her medications or services rendered to reflect their payment, so the consumer is uncertain how much they have been paid.

Because of their disorganization, the consumer has been off her medication. Given the difficulties the consumer has had, she feels as though they are not satisfying the services they are obligated to provide.
