This letter is in response to the complaint that you received from Mr*** *** regarding hisclaim for Short Term Disability (STD) benefits under the above PlanYour notice of September12, was received in our office on September 12, 2016.Please be advised that we have received a duplicate
complaint from the New HampshireInsurance DepartmentOur response will be sent to the consumer through the New HampshireInsurance Department.In response to your request, we provide you with the following explanation of the circumstanceson this matter:Please be advised that Symetra Life Insurance Company is the claims administrator for theVelcro USA, IncShort Term Disability Plan.On June 29, 2016, we received notice of Mr*** claim for Short Term Disability (STD)benefitsOn June 30, 2016, his STD claim was approved for benefitsHis STD benefitscontinued to be issued during the adjudication of the claimBenefits were issued throughAugust 25, 2016.On September 1, 2016, we advised Mr*** by telephone that updated medical recordswere needed to extend his benefits beyond August 25, At that time, we sent him (byelectronic mail) an Authorization form that was required by the physical therapy facility torelease his progress notes to usHis completed form was received on September 1, andwas forwarded to the physical therapy facility that same day.On September 2, 2016, we received one progress note from the facilityMr*** contactedour office that day to inform us that he had not received his benefit check dated August 23, 2016(representing benefits due for the period of August 21, through August 25, 2016)He wasadvised by our office to allow for an additional time period for the check to be delivered and ifnot received by then, we would process a stop-payment on the check and reissue the payment.On September 6, 2016, Mr*** contacted our office again to advise us that he had still notreceived the August 23, benefit checkOn September 9, 2016, following a stop-paymentthat was placed on the STD benefit check, a replacement check was reissued to Mr***.The additional requested medical records were received in our office on September 9, andSeptember 12, Based on the medical records received, we determined that the medicalevidence supported Mr*** continued period of DisabilityOn September 13, 2016, Mr.*** STD benefits were extended from August 26, through the STD Policy'sMaximum Payment Duration date of September 24, and he was notified by telephone ofthis detemination on his claimMr*** STD benefit payments, retroactive to August 26,2016, are being mailed out to him on September 14, 2016.We trust that this letter and the attached correspondence provide you with a sufficient responseto your inquiry.If you should have any further questions regarding this matter, please feel free to contact ouroffice at 1-877-377-6773, or you may contact me directly at *** * *** ***
