communicate very effectively with each customer. [redacted] gave a verbal approve on Thursday August 20th  to a project manager and a project coordinator (both head employees) that the handrail was to be installed a specific way. [redacted] approved these final plans and when Friday August 22st came with about 1-1/2 hrs remaining, [redacted] decided he wanted to change his mind again. His manor in letting us know was also very offensive to my staff and created a very unhealthy work environment for them. [redacted] used phrases such as [redacted], I don't give a [redacted] what I told yall yesterday and other offensive language. Upon receiving a call from the same 2 gentlemen and being notified of [redacted]s offensive language with a demeaning tone I made the decision to let [redacted] retain the balance owed of Over $2000.00 and remove my staff from the project. In addition [redacted]s demands for more additional changes were unreasonable and unable to be met. We pulled off of the project with 1-1/2 hrs remaining as [redacted] waited until we were finalizing the project to "change his mind" once more. In conclusion [redacted]s requests were demeaning, condesending and my staff is not to work in an environment that is unhealthy.  There is about 1-1/2 hrs remaining on the project and the material is on site for [redacted] to have a seperate contractor complete the project. He will have $2000 we will not be collecting as well and should be able to have the final things complete as well as save a lot of money. We would rather lose the final payment and [redacted] be able to contact a company of his choice than to be degraded verbally and spoken down to while on his property. distorted view of what has occurred. She was given appropriate care at all times by our office but her future appointments were cancelled after she failed to pay a balance she owed in the generous time frame given.
She was sent repeated billing statements, a collection letter, and multiple calls were placed to her number in a 4 month period to have past due balance paid. During this period, a billing error was corrected but Ms. [redacted] was informed that she continued to owe a balance. This balance was not paid by the specific deadline given and her balance was sent to collections. At this time, her appointment was cancelled 2 weeks in advance, as she was previously informed would occur if the balance wasn't paid. She proceeded to pay her bill 3 days prior to her cancelled appointment (11 days after the deadline) and expected to be seen at her previous appointment period but the times had already been filled by other patients. She was offered another appointment to deal with her "debilitating medical condition" but instead of accepting this offer, she insulted and cursed my staff and affronted my practice. She was discharged from my practice because of this and past similar behavior, as allowed per an agreement she signed with our practice. Since this discharge, she has commenced to inappropriately threaten our office repeatedly as your complaint notice displays.
OFFER:
Her medical records will be mailed to her home after she pays the fee allowed by the State of Indiana. Further, if she desires the records to faxed to another medical provider, this can be completed without cost for continuation of medical care. The records will be sent to the requested provider in their entirety. This has also been repeatedly communicated to patient.
