The problem with the [redacted] eligibility has been resolved thru his employer. All benefits have been reinstated.  If the [redacted]s have unreimbursed medical expenses they can email them to me and they will be reimbursed.

[redacted] employer reported him as inactive the week of 12/14/15. Coverage ends Saturday of the week you are first reported as inactive as spelled out in the Summary Plan Description provided to [redacted] He has indicated he has received his [redacted] documentation. If he feels that he has incurred...

inappropriate medical expenses that cannot be resolved by [redacted] or if his employer owes additional contributions due to accrued vacation the Fund has a two step appeals process. He can obtain an appeals form at [redacted] If he has additional questions he can contact me directly.
