(Revdex.com Copied from original document.)This letter is in regards to Case # [redacted] Mr [redacted] On June 1, 2016, I met with the owner and examined King, a 8-year old male neutered golden retriever belonging to [redacted] , just weeks after they had adopted himBased on King's medical signs, reported by the owner, and a physical exam performed by myself, I made medical decisions and recommendations based on his current medical statusThe complete blood count and chemistry parameters were within limitsThere can be underlying medical conditions which cannot be detected even with a thorough physical exam and review of medical signs.When King's medical status changed, the owner presented him to the emergency hospitalAt that time, King was experiencing new absigns and new abphysical exam signs were detectedI was able to speak with the attending emergency clinician who handled King's case and a cardiologist who reviewed King's caseThey expressed that, in this case, King's rapid medical decline and underlying medical condition could not have been detected at the time of my exam of King Mr [redacted] was given copied of all laboratory results that were performed at out hospital and he received detailed reports from the emergency hospital, Metropolitan Veterinary HospitalIf the Revdex.com would like copies of those results we would be happy to include themThe day after King was presented to the emergency clinic, I was able to speak with Mr [redacted] by phone and discuss his concernsI expressed my sympathy and advised owner that I would credit his account for the heartworm preventative that he had purchased at the time of King's visit that he had not yet given to KingThe following Monday I received an email from Mr [redacted] , I have attached it as well as my response and his additional responses.I was very saddened to hear of the loss of KingThe owner made a significant emotional and financial commitment to adopt and care for himThey recognized that King was not being cared for by his previous owner and they decided to lovingly help him.King was fortunate to have been so loved and cared for by his new owners, even though it was such a short time.We at Tender Loving Care Animal Hospital strive to provide the highest quality medical care with compassion and integrityWe are dedicated to the physical health and emotional well being of animalsWe strive to ensure that we all share the joy and unconditional love of pets for a lifetimeWe are very sorry that one of our clients is unhappy with our servicesThe client is asking for a refundThe charges that this client is asking to be refunded are for services we renderedWe are very saddened by the news that King has passed awayWe believe that a thorough exam and diagnostics were performed

I never received any copy of any blood work from themSecondly while at the Metropolitan Clinic I was told the exact opposite that King conditions were obvious to them as is should have been obvious to TLC as we'llHeart Disease is a common occurrence in Large breed dogs which King wasX-rays were never recommended yet needed and should have been the first thing that was recommended but was notInstead I spent $on test that proved nothing or test they said they performed
With that in mind I took King to an emergency clinic days lateronly to lose him to the conditions mentioned aboveI
I don't ever expect any one person to be perfect but to make me pay for you unsound diagnosis is extremely unprofessional and makes me a victim twiceYour diagnosis was not accurateHe was not in good HealthSecondly heart disease does not show up days laterYou know heist on keeping these wounds fresh in my mind by having to defend the Honor of my pet as your medical care you gave to him only worsened him
I'm not satisfied with your explanationIt's all a bunch of words to me

Case #- Mr*** ***On June 1, 2016, Mr*** presented King, an 8-year old male neutered golden retriever mix that he had adopted just months prior from a friend who wasn't providing the care he deserved, according to Mr***
Mr*** presented the pet for a physical exam and any needed vaccinationsHe agreed to vaccinations and blood testsI performed a complete physical exam, administered vaccinations, and performed the blood testing that Mr*** had agreed upon(Please see the attached blood test results and a copy of Mr***'s invoices for services provided.)
Given King's sad set of circumstances, I respectfully agreed to refund Mr*** the cost of the heartworm preventative (Heartgard) that he had not yet administeredI asked that he dispose of the un-used medication himself in accordance with the Ohio State Board of Pharmacy law that states that I cannot accept back any medications that have left the premisesI also refunded him the price of the pre-paid urinalysis test, the sample of which he never had the opportunity to return to usOur medical records and email records confirm that we did email Mr*** a copy of the results of King's blood test, but we are providing another copy today
Mr***'s account at Tender Loving Care Animal Hospital currently has a credit for $We still need him to provide us with his Care Credit card information so we can credit that account
I was very saddened to hear of Mr***'s loss of KingHowever, we did provide the services on June 1, for which he paid Tender Loving Care Animal Hospital
