This letter is in response to the allegations against Texas Certification Board of Addiction Professionals Office reference # *** filed by *** ** ***We hope this clarifies the issues she has
complained about
When a Candidate pre-registers to take an exam with TCBAP there is a process that is given to them in writing that they are required to followIt is as listed:
1) Send in Pre-Registration Form, copy of LCDC-I license, $Test fee
2) Receive Conformation Letter and Email (contains registration, steps) "See Attached Letter"
Once one has registered online for the exam they must make sure all their information i.e(Name, Address, DOB, SSN) is correct
Once the test has been taken, if the candidate fails they are sent a letter along with their test scores that explains:
Candidates who believe an unusual event or condition related to the administration of their examination caused a significant adverse effect on their performance during their examination may submit a grievance regarding the exam administration to IC&RC for investigation
Grievances must be submitted to IC&RC within calendar days of the examinationGrievance statements must be submitted in writing, dated, and signed Grievance statements must be accompanied by the IC&RC Exam Administration Grievance FormInformation to include in the grievance statement should include, but is not limited to:
Title of exam
Examination date
Name and location of testing center
Name of proctor on duty (if known)
Detailed explanation of the situation
Impact the situation had on exam performance
IC&RC will then investigate the specifics of the testing situationWhen warranted, candidates will be offered a free retakeAn exam grievance will not challenge the design or content of an examination nor overturn a failing scoreThere is a $fee for this serviceCandidates that are offered free retakes will be refunded their grievance feePlease contact
*** or ###-###-#### for a Grievance form"See Attached"
Allegations in regards to *** ***, We do not show that she followed the correct steps in filing a Grievance with TCBAF' or IC&RC, as she emailed IC&RC directly with no Grievance Form or Fee"See attached email"
Per our policy there is a $testing fee each time you sit for the exam
In order for TCBAP or IC&RC to investigate a grievance and verify anomalies, the grievance procedure outlined in the policy that the candidate receives is required to be followed

THIS COMPLAINT SHOULD ALSO BE FOR   I SO QUALITY TESTING, INC  IS USED TO GIVE TEST TO APPLICANTS
THEY ARE RESPONSIBLE FOR PROCESSING ALCOHOL AND DRUG COUNSELOR TEST SCORES (ADC EXAMINATION.
SOME ONE IN THERE OFFICE IS TAMPERING WITH APPLICANTS TEST SCORES. I BELIEVE TEXAS CERTIFICATION BOARD RESPONSIBILITY
IS TO MONITOR THEM AND MAKE SURE THEY ARE PRACTICING RIGHT PRACTICES IN COMPUTERIZING APPLICANTS FOR LCDC LICENSE TEST RESULTS.
HOW DO I KNOW IF THEY HAVE DONE THIS TO SOMEONE ELSE. I HAVE TAKEN THIS TEST OVER AND OVER AND SCORES ARE NOT MATCHING UP.
