I noticed we show a different address for you in our computer system —[redacted], Cedar Park, TX [redacted]. This is the address on your scanned driver's license, too. I am not certain if that is where the breakdown of communication originated, but there is a discrepancy nonetheless. Also. I must apologize for the delay in responding to your letter. The address used in mailing was for our old bank lockbox that we have not used on our statements in over a year. Thankfully, the bank forwarded your letter to our current address. I have included the Explanation of Benefits (E0B) from United Healthcare for each of your dates of service indicating the patient responsibility.
