Ms [redacted] owes unpaid taxes to MassDeptof Revenue (DOR) and her account was placed with us on 8/21/A phone message was left on 8/25/and we spoke with taxpayer (TP) on 9/29/when she said she was not working and could pay $per monthA formal payment arrangement (PA) was set up and a PA agreement letter was mailed on 10/Phone messages were left 10/and 10/On 10/27, TP said never received PA and another was sent on 10/Phone messages were left on 11/3, 11/10, 11/16, 12/and 12/since she had not returned the signed PANo response to the messages was receivedOn 2/3/we assisted TP to pay $on the DOR websiteNo funds are paid to us; any payments are paid directly to DOROn 2/3/we sent our third PA letter because the dates had to be changedOn 2/10/16, we left another phone messageOn that same day, DOR recalled the accountWe no longer have the accountOn 3/28/2015, Ms [redacted] called to complain about enforcement actions put in place by the MassDepartment of Revenue due to her unpaid taxes after the account was recalledPlease note that Ms [redacted] never returned a signed Payment Arrangement letter, even after three had been sentMs [redacted] 's complaint has nothing to do with ADA and should be expunged from our record
