I am rejecting this response because: These all symptoms that doctor described they were "known" and I knew about it. Also I clearly mentioned that I wanted eye exam for new prescription. For new patient routine diagnostics, it did make sense as this was not the first eye exam for him, he has been done eye exam many times every years. Any way if doctor saw those, he should have asked me for permissions before start doing any things. I felt like that the doctor did diagnoses over my permissions and my consent. Also after searching around on website relating to this provider practices, I found out that I'm not the only one who has this satiation. I'm seeing another patient getting the same practice as me. This indicates that it has been happening many times.

To whom it may concern,Mrs. [redacted] brought her son in for an eye exam.  During this exam it was identified and documented that her son has “itchy/scratchy eyes from time to time”  Additionally the exam revealed +2 papillae of palpebral conjunctiva of both...

eyes which is consistent with seasonal allergic conjunctivitis.   Pazeo one drop to each eye every day was prescribed for the relief of conjunctivitis symptoms. During the evaluation, the child’s vision was also assessed and a new prescription for glasses was provided.This was a first time visit for this particular patient, and as with any new patient a complete and thorough exam is provided.  The visit was submitted to insurance which paid on the vision portion of the exam, but the medical portion went to deductible.  This claim was processed correctly.  Should Mr. or Mrs. [redacted] not agree with the doctors diagnosis, they can appeal the claim via his insurance company.  The insurance company will then request chart notes for review to determine if the coding is appropriate.  Mr. [redacted] writes that “I didn’t ask for any diagnose from the doctor.”  Mr. [redacted] also writes that he wanted his son to “get his updated prescription”  In order to provide a prescription there will always be a diagnosis.  In addition to the conjunctivitis, his son also has myopic astigmatism.  We submit all findings to insurance companies as we cannot leave off a specific diagnosis for the sake of better payment by insurance.We would be happy to submit the chart notes as requested for a review.
