The Dental Care Center

The Dental Care Center billed me for a fluoride application which was not authorized by me or the insurance company([redacted]).On 9/18/15 I brought my daughter in for her routine dentist appointment. After she was done we left the dentist. Not one dentist or any receptionist told me that she had a fluoride application or that the application wasn't covered under my [redacted](#[redacted]) insurance and I would have to pay out of pocket. My insurance covers fluoride applications once a year which The Dental Care Center should have known because they did her first fluoride application on 3/17/15. Also she had two different dentist on her appointments. [redacted] on 3/17/15 and [redacted] on 9/18/15. [redacted] attempted to bill my insurance for the fluoride application but my insurance didn't pay for it because they only cover 1 fluoride application per year. So either the dentist didn't look at my daughters chart and insurance coverage or the dentist purposely tried to get file a false claim to pay for a procedure that was not warranted. After I got the bill (22.00) in the mail I went back to the The Dental Care Center to question the bill which I didn't approve or authorize any dentist to proceed with any fluoride application they told me there was nothing that they could do. They had no problem telling me what my insurance did and did not cover when it came to "my" dental procedures. Now the bill is in collection and its damaging my credit and FICO scores. I also have a copy of both claims proving that their were two insurance claims filed for fluoride application.Desired SettlementI want be cleared of the responsibility of the fluoride application bill (32.00).A cease and desist from the [redacted] collection agency. And a correction on my credit report.
