July 14, Dear [redacted] came to the office on March 20, complaining of needs for dental treatment to reverse what her previous dentist had “wrongfully” had done to herNecessary diagnostic X ray were taken and various treatment options were discussedThe patient was adamant on fixed restoration of her missing teeth baring bridges and all necessary preoperative instructions were discussed including the need to stop smoking when we placed the implants inShe accepted totally and promised to complain with all the post-operative instructionsOn March 24, two implants were placed on the lower right jaw and sutures were placed as necessaryMedications were prescribed for pain and to curb any infectionPost-operative instructions were also givenThe instruction not to smoke was particular emphasizedhour post-operative review was satisfactory and patient was instructed to call if there were any problems.By March 31, the sutures were removed following satisfactory evaluation that the healing was progressing property.April 15, patient came in for the placement of two implants in the left lower jawSame pre and post-operative procedures were duly followedBy April 27, post-operative review was satisfactory and patient was asked to continue on post-operative mouth rinses and warned against smoking again May 6, patient came in complaining of facial pain from the lower left jawExamination showed that all the sutures placed had broken down and the implants completely exposedDebridement was done After surgery patient was confronted with the fact that she had been smoking heavily, it was obvious from her smellShe denied completely of using tobacco and eventually agreed to cut down on the number of sticks per day that she was takingShe attributed her smoking to the family stress she has been involved inMAY 9, the pre-operative review was satisfactory, pain experience was downMay 15, a review showed the mucosa on right side of implants was swollenPatient was then made to be aware of what would possible happen if continued to defy instructionsJune 4, patient came to the office with her friend to tell us that they visited another Dental Office and they were told that she had no more bone in her jaw, demonstrated to the patient that her jaw was still there but would need to help the process by stopping the smokingOn June 28, patient called to request the removal of all the four implants and want her money backApparently [redacted] and friend went to an Oral Surgeon for a consultationThe oral surgeon gave me a call that [redacted] oral hygiene was bad and she had been smoking heavilyHe advised that base on radiograph taking and the culture of the patient, the implants should be taken out since she was non-compliant with post- operative instructionsJune 29, the implants were surgical removed, Patient requested all of her records with us and were given to her.July 7, a letter from Revdex.com arrived at this office, [redacted] denied frantically that she had no knowledge of who did itShe requested to be treated post-operatively asking that she was open to further discussion on her treatmentJuly 8, patient complain of pain from the sites where the implants wereOral and post-operatives instructions were givenPatient needs to stop smoking and she was told several timesThanks for the opportunity and please contact this office if you need more informationYours Cordially, Victor F DMD PhD Dentist
