July 14, 2015Dear [redacted] The above mentioned patient was first seen in this office on March 31, 2015 brought in by her friend [redacted] ***. Comprehensive medical and dental evaluation; were carried out, Necessary diagnostic radiography were taken. Her chief complains... were severe headache and bad smell coming out of her mouth. She smokes and promise to quit once we start treating her. Various treatments options were discussed but patient was disgusted with any other form of treatment instead of implants. From the clinical and radiographic evaluation the upper left treatment was to be treated first after general mouth cleaning.There were four teeth that had severe periodontal diseases with multiple cavities. At the consent of the patient treatment plan was written down to be reviewed by patient. An April 17, 2015 patient came in and four of her teeth were extracted followed with debridement and the placement of two implants. The patient was given adequate post-operative instructions and medication for pain and infection. She was instructed not to smoke during the course of the treatment. She promise to comply, Patient was to return for post-operative evaluation a week if everything was fine, May 29, 2015 patient came back for post-operative review. Healing was satisfactory. All the sutures were removed and patient were assumed that her headache was not from implants that were placed as she has always been having headache before the implants were placed. She was advised to stop smoking. May 12, 2015 patient came in company of her friend ( [redacted] ***) and complain again of headache. She was ask to see her primary physician as everything else showed that implants were doing well. Asked to stop smoking, she told us that in honesty she cannot stop smoking. Immediately she was advised on the result of her noncompliance to stop smoking during oral surgical procedures, Having gone in the company of her friend [redacted] to the oral surgeon, she was told that ever though she had bad oral hygiene and smoke heavenly, her implants were well osteointegrated. June 28, 2015 patient called the office asking for her implants to be remove and refund of her money. She was ask one more time to see her primary physician before we do anything for her. Patient therefore demanded her records.Characteristically she would appear in the office without an appointment and demanded her records, money and removal of the implants. Dear [redacted] came to the office on March 20, 2015 complaining of needs for dental treatment to reverse what her previous dentist had “wrongfully” had done to her. Necessary diagnostic X ray were taken and various treatment options were discussed.

patient was adamant on fixed restoration of her missing teeth baring bridges and all necessary preoperative instructions were discussed including the need to stop smoking when we placed the implants in. She accepted totally and promised to complain with all the post-operative instructions. On March 24, 2015 two implants were placed on the lower right jaw and sutures were placed as necessary. Medications were prescribed for pain and to curb any infection. Post-operative instructions were also given. The instruction not to smoke was particular emphasized. 24 hour post-operative review was satisfactory and patient was instructed to call if there were any problems.By March 31, 2015 the sutures were removed following satisfactory evaluation that the healing was progressing property.April 15, 2015 patient came in for the placement of two implants in the left lower jaw. Same pre and post-operative procedures were duly followed. By April 27, 2015 post-operative review was satisfactory and patient was asked to continue on post-operative mouth rinses and warned against smoking again. 
May 6, 2015 patient came in complaining of facial pain from the lower left jaw. Examination showed that all the sutures placed had broken down and the implants completely exposed. Debridement was done.  After surgery patient was confronted with the fact that she had been smoking heavily, it was obvious from her smell. She denied completely of using tobacco and eventually agreed to cut down on the number of sticks per day that she was taking.
She attributed her smoking to the family stress she has been involved in.
MAY 9, 2015 the pre-operative review was satisfactory, pain experience was down.
May 15, 2015 a review showed the mucosa on right side of implants was swollen. Patient was then made to be aware of what would possible happen if continued to defy instructions.
June 4, 2015 patient came to the office with her friend to tell us that they visited another Dental Office and they were told that she had no more bone in her jaw, demonstrated to the patient that her jaw was still there but would need to help the process by stopping the smoking.
On June 28, 2015 patient called to request the removal of all the four implants and want her money back. Apparently [redacted] and friend went to an Oral Surgeon for a consultation. The oral surgeon gave me a call that [redacted] oral hygiene was bad and she had been smoking heavily. He advised that base on radiograph taking and the culture of the patient, the implants should be taken out since she was non-compliant with post- operative instructions. June 29, 2015 the implants were surgical removed, Patient requested all of her records with us and were given to her.July 7, 2015 a letter from Revdex.com arrived at this office, [redacted] denied frantically that she had no knowledge of who did it. She requested to be treated post-operatively asking that she was open to further discussion on her treatment.
July 8, 2015 patient complain of pain from the sites where the implants were. Oral and post-operatives instructions were given. Patient needs to stop smoking and she was told several times. Thanks for the opportunity and please contact this office if you need more information.
