[redacted] , I am writing in response to the complaint submitted on 01/30/The patient was seen on 11/28/There was only one treatment plan offered to herIt is The Dentistry’s policy to follow up with any patient that was seen by one of the doctors and the doctor recommended treatment We follow up with a phone call on 01/03/18, then a letter on 01/18/ and then another phone call on 02/08/we then sent a dismissal letter on 02/26/In the patient complaint she stated she ask us to remove her from our data base she never stated this to the office If this was stated we would have sent out a letter dismissing her from the practice and inactivated her accountWe have legal obligations to follow up with any patient that treatment was recommended and we document this in the patient recordsPlease see attached letters sent out on 01/18/and the dismissal letter on 02/26/Since no one ever spoke to her we only left two messages and only one treatment plan was offered does she have us confused with another dental office? Please let me know if you see the first letter we sent out demanding in any wayThank youThe Dentistry

treatment.  We  follow up with a phone call on 01/03/18, then a letter on 01/18/ 2018 and then another phone call on 02/08/2018 we then sent a dismissal letter on 02/26/18.
