Review: I registered to take the NPTE (Nation Physical Physical Therapy Examination) on October 9, 2013. I arrived at my appointment time early, and was asked to present my two forms of identification that was clearly required . On the FSBPT website, it states that in order to test at any Prometric Testing Center, two forms of identification were required. One needed to be a driver's license, and the second must have a pre printed name and a signature, (e.g. credit card). I presented my driver's license and a credit card, but the credit card had my maiden name on it. I was told that both ID's must have the same last name. I realized that I had missed this while reading the instructions. I then presented my Sam's Club Card, which has a pre printed name, a signature, and a photo of myself. I was denied sitting for the exam. I called the FSBPT and was told that club cards were not acceptable. I would agree with this decision if all criterion were not met. However, my club card met and exceeded requirements for a second identification. Not only was I not able to sit for an exam which I studied for for over 2 years, I am not eligible to receive a refund of my testing fee of $370.00. My appointment time was for 1:00 pm. I was scheduled to test until 5 pm. When I asked if I may present another form of I.D. at 2 pm, I was denied.

My issue is this, I do not have a credit card. I do not have a passport. I was told that my student ID card would be sufficient. I did not bring it to the testing center because it does not have a signature.Desired Settlement: I would like to my fee to be transferred to the next testing date.
