On February 27, 2017, I visited University Dental *ssociates for an initial dental preventative visit. During the process, I was examined by Dr. *. [redacted] , DDS. Upon leaving the office, one of the assistants told me that I needed crowns on 2 of my teeth #s 14 and 15, as well as to redo a filling on tooth 18. I specifically told her to make sure my insurance will cover the cost of the crowns, which meant the dental office was required to get a pre-authorization before doing the work. The lady that I spoke with on that visit, who was at the front desk working, told me not to worry about it because she was sure they will pay for their portion of crowns. I said to her, "ok, so long as you are sure, thinking she would get the authorization from my insurance companies. I had 2 dental insurance coverage, [redacted] and [redacted] . Then on *pril 03, they placed temporary crowns on my teeth until the permanent crowns came in and were delivered in my mouth about another 3 weeks later. Then

During 2015, I sought the services of this dental office I had a cleaning done on my Upper Left Quadrant and paid the copay required They filed the claim with my insurance company who paid the claim in full except for the cleaning itself because the office filed the claim as "IL" instead of "UL" (Upper Left) I have called at least times to get this rectified and my insurance company has not received the appropriate information in order to settle the claim
I understand that the claim is not a huge amount, but the hassle of trying to get the claim paid by my insurance (which would be covered 100%) is infuriating I have taken time out of my work day to call them and explain the situation and they promise to rectify it On top of this, they send me letters with late charges accruing even though each associate I have spoken to insists they will take care of it I do NOT want it hitting my credit report for a mistake that could have easily been rectified months ago by the o

I went there for a crown During the anestesia, they use the wrong anestesia and I had a reaction to the anestisia that I could hardly breath, I began to see everything upside downthe practice called and took me to the hospital in an ambulance, the doctor determine that I had a reaction to the anestesiaI have been calling the practice lately and they stated that I didn't receive any treatment there, that the doctor and the business manager at that location are new and I asked for the business manager to called and pay for my bills They don't even return my phone calls

My son was treated by a dentist at this location for two yearsThat dentist is no longer thereHis practice has been handed over to another dentist and the new dentist is unaware of the agreement we had with the last dentistI have explained the situation more than one time and they still insist on billing me and now have sent the account to a collection agency called xxxxxMy son was working with an oral surgeon along with University Dental AssociatesThey had to preform surgery on his front tooth that had been knocked out during a brutal attachThe bone graphing and permanent placement of a composite tooth did not workThe doctor said that in all of his practice he could only remember a very few times that this had happenedTherefore after many months of pain and imposition, the entire procedure had to be repeatedNew bone graphing and another year of waiting for it to adhereBoth Doctors ASSURED me that there would be NO additional costs on top of the dollars we had a
