The director of this clinic discharged me from this clinic without the doctors or my knowledge Dr [redacted] took care of me at Methodist center and kept me alive again, this last time he told me to come in right away and see him for a follow up from the hospital and the people at the office had told me I can no longer go to this officeI was never told anything before nor did Dr [redacted] know about any of this Dr Hicklin wrote me back and said it was not his decisionplease help I would love to go back to see him Product_Or_Service: services

I RECEIVED AN INVOICE FOR A SERVICE THAT WAS NOT PROVIDED TO ME OR MY DEPENDENT THE IOWA CLINIC INVOICED ME FOR A SERVICE I OR MY DEPENDENT DID NOT RECEIVE I ASKED FOR A COPY OF THE ADMISSION PAPERWORK AND THEY DO NOT RESPOND TO MY REQUEST I FEEL THIS IS IDENTIFY THEFT THEY ALLOWED ANOTHER PERSON TO RECEIVE SERVICES AND PUT THE BILL IN MY NAME AND USING ANOTHER PERSON'S ADDRESS I RECEIVED A PAST DUE INVOICE THAT COULD HAVE LED TO COLLECTIONS AND DAMAGING INFORMATION REPORTED TO THE CREDIT AGENCY THEY SHOULD HAVE A POLICY THAT AN ID IS SHOWN FOR SERVICES AND BILLING SO THIS ERROR DOES NOT HAPPEN

Iowa Clinic repeatedly failed to properly file my work comp case, resulting in fraudulent billing of BC/BS and me, and sending bills to collections
Iowa Clinic Neurosurgery Dr*** saw me on 7/24/for an approved work comp injuryAppointment established with extensive discussion of work comp status, and work comp approved claim # was provided at each Iowa Clinic visitDr*** recommended surgery; pre-surgery visits ensuedPella Clinic & Hospital did several pre-surgery evals & properly filed work comp claims and were paid Iowa Clinic did not properly file work comp claims for 8/5/cardio work up & were not paid; subsequently billed my insurance BC/BS who did pay (but should not have done so on 9/24/14)Iowa Clinic tried to bill BC/BS again on 12/16/14, 7/16/15, 11/11/15, 3/15/16, 5/31/& 6/29/16-all deniedIowa Clinic tried to bill work comp on 1/9/and 6/22/16-denied due to failure to do pre-approval and the case being expired on 12/31/ I sent letters and form

Received retroactive Medicaid but have not been reimbursed for payments made in 9/

My daughter started receiving Medicaid (2948710E) in 11/2015, which was made retroactive to 8/1/in 12/
Contacted Iowa Clinic asking them to file for Medicaid for bills back to 8/1/on 12/14/
Also followed up on 1/14/2016, 2/15/2016, 2/25/2016, 3/21/2016, 4/13/2016, and 7/6/ Received partial reimbursement but not major portion of bill for 9/3/
Iowa Clinic blames Medicaid for not paying but also admit to not providing all requested information Medicaid confirms problem is with Iowa Clinic Iowa Clinic does not wish to pay what they know they should until reimbursed by Medicaid but Iowa Clinic has not taken initiative to correct filed claim Over and over, Iowa Clinic billing politely promises to get it resolved but apparently not committed to getting problem solved The bill was initially paid

A fee was billed and paid before services rendered, the service never occurred and I want the fee returned
The service that, required a fee of $1,up front, was for a deposition by my doctor (DrBarbara *** they required this fee be paid at least a week before the day of the depositionInstead they requested it from my attorney almost two weeks before the scheduled depositionThe deposition was cancelled however DrHodne still showed and was told that she was not needed that a settlement had been reachedThe $1,fee was for a deposition and the deposition never took placeI have tried, on numerous occasions, to contact the office manager at the Iowa Clinic and have been unable to reach him or her(There are two different office managers) I have left numerous voice mails and I had even left one message that included one of them to leave me a message stating when a good time for me to call them might beThey did leave one voice mail but did not state a good time for

*** *** never bothered to read my son's medical records we transferred over before his appointmentAfter I detailed his entire history up to the current information in person at the appointment, she recommends a second appointmentI make itShe calls FIVE DAYS AFTER OUR FIRST APPOINTMENT saying she can't help usShe never answered correctly when I called to make the first appointmentShe never bothered to read my son's chart I wasted time on transferring to her PRIOR TO THE APPOINTMENTShe never LISTENed DURING THE CONSULTATIONI literally explained what we were going through with my son, showing photos of the timeline to her at our first appointment for her to call days after that appointment and say "I didn't realize you were..." She missed that information the THREE TIMES I GAVE IT TO her IN THREE ENCOUNTERS
I called the administration office and spoke
