On Tuesday, February 22, 2017 at 3:30 PM I had a conference call with [redacted] (Lafayette Life), [redacted] (Lafayette Life), and [redacted] (Capital Financial Partners) to discuss resolution of my complaint. It was agreed by all parties that Lafayette Life will allow me to surrender my contract and policy at the end on my annual renewal period which ends on May 15, 2017 and allow the account to accrue any market performance earnings that would then be credited to my account. We agreed that Lafayette Life will WAIVE the surrender charge penalty and after the credits (if any) are applied to my account, will then terminate the contract and send me a check for the full amount in my account with NO SURRENDER CHARGE PENALTY WITHHELD. This agreement is to be provided in writing and sent to me via email no later than Monday, February 27, 2017 for my review and concurrence. I am awaiting the agreement to review and acknowledge. I will consider this compliant still open until such time as the agreement as stated is verified, confirmed, and signed/approved by all parties involved. Regards,[redacted], February 24, 2017

One of our Lafayette Life representatives spoke with [redacted] in January and let him know we needed our information request form that was sent to him in December 2015 to be completed and returned to us for those calculations to be provided to him.
With respect to not answering his calls,...

it seems that two of the three calls that were made were made by a third party on his behalf. On those two occasions the individual was told we could not release information to him as there was no written authorization to do so.

I was to received an agreement document outline the terms to resolve this compliant. As of today, March 6, 2017, I have not received any correspondence regarding the agreement decision with Lafayette during a conference call on February 21, 2017 @ 3:30 pm. Below is the text of an email I received on February 27, 2017 describing when the cover letter and terms of the agreement were due for my review. I have not received such documents and consider this compliant OPEN until such agreement document has been received, reviewed, acknowledged, and approved by all parties>From: [redacted] Sent: Monday, February 27, 2017 3:33 PM To: '[redacted]' Cc: [redacted] Subject: RE: Follow up from phone callMs. [redacted]I wanted to give you a status update. I will be sending a cover letter to you no later than end of day tomorrow detailing what we agreed on. This will also let you know that we will be sending a settlement agreement for you to sign in May after the index accounts are appropriately calculated and credited. The settlement agreement will have the exact dollar amounts in them so you can know exactly what to expect as a result of the termination of your contract and waiving of surrender charges.Please let me know if you have any questions in the meantime.Thank you
