They said they filled (2) teethThey lied & I can prove by showing you the teeth they claim to have filledThen send me a bill for $This matter needs to be seriously looked into This is "FRAUD"

Patient *** *** (I.D# ***) came into our office on 10/17/2014, in this visit we recommended dental treatment including fillings, crowns, extractions & partialsWe mentioned to the member that services "would" be covered at 100% under the existing plan however her ins
coverage changed in the beginning of 2015, this is why she ended up with cost's for her dental treatmentMember had fillings done on 1/28/for teeth #& #at this time we were unaware of the insurance changes, so we had not charged member for these servicesOn 02/23/we got notification from insthat coverage had changed and patient had copayments for tx nowWe then Called the member and informed her of this but she still wanted to schedule another appt., we also tried collecting the past due amount for the fillings that were done on 1/28/but member said to bill her for the servicesWe told her we needed to collect the amount before seeing her and started yelling and screaming with foul wordsAt this point in time we decided to dismiss member from the practice. *Fillings were at a cost of $, crowns at $& partials at $each. If there is to be any other questions or concerns in regards to this patient, please let us know thank you

my rebuttal to modern dental regarding my complaint id is as follows: recalling events backwards on January 29, I called modern dental because the entire tooth including the temporary crown intact came out of my mouthi did not call to have it extracted, it had already broken offno one told me that a redo of the crown must be done, how can they offer to redo the prep for the crown at no charge when theres no tooth to be preppedbut other services was offered which again my medical plan doesn't cover, at the price of $on upmy rebuttal is there should have not been a root canal done, the services offered now should have been offered in then I saved the money for the crownnow I dont have that kinda money and the tooth isn't intact to be fitted for a crown again anywayso therefore I am requesting my refund of in which they are refusing to returnAny more infoPlease call *** ***Please note: message attached

have to probably this crown prep for now it is broken down further (we could have charged her for the lab needs to remake the crown)Now she wants it extracted The cost for the crown on #(front tooth) is very low, $which she only paid a deposit of $Her MediCal plan pays $on crowns so we try to keep cost lowThis is on her ledger attached I have also attached the informed consent for her crown which she signed.We document everything when it comes to patients and she never filed a complaint with her MediCal plan, Liberty/Health Net for they would have contacted us first Please let me know if there is anything else. *** ** ***Hayashi Dental Group***Office ~ ***
