The Performance Center is a healthcare facility, and complete patient charts are not available to non-medical personnel...

(like me). However, in reviewing a patient list so that I am able to have the medical staff at the Center provide me with an explanation, I can find no patient named [redacted].
Further, when a patient pays for services, he receives what we call a Superbill which does not include the word Invoice or reference any number or code.
And, finally, it is not a normal practice for medication to be "sent" to someone. Long-time patients can be mailed supplements and refill items under certain circumstances and upon special request. We do track shipped items closely, however, and I show no record of anything being mailed to the consumer's address recorded in this complaint.
Please ask Mr [redacted] to provide copies of the "Invoice" he refers to and possibly even the "other man's" received medication so that I can locate the proper patient chart(or charts) and provide a complete response.

I'm sorry I was out of town and did not have enough time to respond. My compliant has NOT been resolved.
Here I'm responding to Ms. [redacted]' claims:
1. My name: [redacted]
2. Invoice# XXXXXXXX Code# XXXXXX see picture attached.
3. Medication obtained from [redacted] at [redacted]'s place of residence in Desert Hot Springs with my name on it and a name of some other man. [redacted] told me to take the medication that had the label with someone else's name on it, because in his words "it's the same medication anyways.." I chose not to. see pictures.
4. [redacted] is one of the "professionals" I spoke with primarily.
I'm asking for a full refund of my money and an investigation into this "company's" HIPPA related practices.

Upon our receiving the requested information, I was able to locate a chart for [redacted] and our Superbill dated Dec 27, 2014 matching the dollar amount of his sale.
After researching Mr [redacted]'s claims with other members of the medical staff, we believe the accusations he makes, with some variation, are foundationally true. The Manager of this Center has been discharged. I will be mailing a full refund check for $649 to Mr [redacted] at the address he provided in his intake documents. That check will go out on Tuesday, June 30, with our apologies.
