To my recollection there was no contract offered I was introduced to Mr [redacted] at his seminar and made an appointment for subsequent meetings No idea what correspondence you need, most was verbal but I do have written proof of his signing me up for Medicare supplement that I was not eligible for, that he later promised to reimburse me for but never did I'm currently working with insurance company to get my money back

January 20, On March 5, at 6pm, *** *** attended an educational Medicare workshop held at the Community Works Conference room. In this workshop, we provide general information about Medicare obtained Medicare’s public website at www.medicare.gov. This website was provided at the workshop Mrs*** attendedMs*** provided positive feedback on the workshop evaluation. Mrs*** contacted me and wanted to set up a meeting to discuss Medicare. We met on Thursday May 14, at my office. We went back over the Medicare parts and the different enrollment dates and the different Medicare gap coverage Ms*** stated in the meeting the she had not declared when she was planning to retire from her employer. During this meeting she asked to know more about social security therefore I told her she would need to contact social security to get the facts She said she would contact me once she had a better idea of when she was going to retire. On June 29, at 6pm, Ms*** attended another educational Medicare workshop held at the Greenville Hospital Patewood conference room. This workshop was conducted in the same manner as all our Medicare workshops and the same information was provided to all (6) of attendees. Ms*** left this workshop before the end of the session which normally last about one hour and minutes. Ms*** did not provide any feedback on this workshopMs*** contacted me again and set up an appointment on August 11, to discuss Medicare and her options to cover the Medicare gaps. She stated that she was going to retire in October of 2015. She agreed that she wanted to sign up for a Medicare supplement that would begin on October 1, 2015. We completed and she signed an application on August 11, 2015. I will note that on the signed application there is a section that indicates that the applicant will be starting both Medicare Part A and Medicare Part B. These dates were provided by Mrs***Ms*** stated in the meeting that she was going to wait to enroll into Part D of Medicare. She wanted me to contact her to discuss her options. I contacted her in October of appointment on November 3, to discuss her Part D plan options. She was unable to stay long at this appointment, but she changed her mind and decided to pick a different option for cover her Medicare. She wanted to do a Medicare Advantage plan and asked if was ok for her to take the application and complete it. She took the application and we rescheduled for November 20, Her coverage for the Medicare Advantage plan would start on December 1, 2015. I collected the application for her and review the information and noticed that she didn’t have a Part B effective date listed on the application. I asked her and she immediately stated to me “I was never told that I had to do that”. I will note that Mrs*** attended two workshops on two different dates where she and all other attendees were provided this information verbally, in an email copy and were referred to Medicare’s public website at www.medicare.gov. She was insisting that the Medicare supplement was all she neededI revisited the Medicare application to confirm that she was stated planning to enroll into Medicare Part B on October 1, 2015. At this time we contacted the Medicare Supplement company to cancel the coverage and request a refund. She was sent a cancellation and refund notice date effective November 20, 2015. Also in the meeting I provided her the phone number to social security to enroll into part B. She left my office seeming to be understanding of the events. On December 9, she contacted me to get her Medicare Advantage Plan paperwork and wanted a full refundIt is unfortunate that Mrs*** feels the way she does and our company takes complaints very seriously. With this said, Mrs*** has a responsibility to take ownership in enrolling in Medicare A and B. This is a function of the government, not our companyWe don’t work for or represent Medicare or Social Security in any way. Nobody from my company every told Mrs*** NOT to enroll and her claim is that nobody told her to enroll. In Addition, our company is a facilitator to getting our clients health insurance from a parent company, we are responsible for making the request for cancellation and requesting a refund on the client’s behalf but, we have no control on the timeliness of the refund. This is an issue Mrs*** should take up with the issuing company. We assisted Mrs*** and provided her with the required documentation. It is unfortunate that this matter has taken anyone’s time. We hope that Mrs*** is able to get what she needs concerning her Medicare. We are available if she wishes to purchase secondary insurance to help her supplement her government sponsored Medicare Plan Kindest Regards, *** ***
