Review: I spoke to [redacted] (my insurance carrier) and was told I shouldn"t have a copay. I was charged an $80.00 copay when registering for my surgery the upcoming Monday. I spoke to [redacted]n billing and she told me my $80.00 would be sent. I called her back and told her I had not received the $80.00. She told me that she had not received payment from [redacted] from the services performed on 1/25/13. I checked with [redacted] again and was told I had no copay. On June 13 I sent an e-mail to [redacted] I told her I had received notification that both Medicare and [redacted] had paid all claims and that [redacted] had paid the balance due on 5/21/13. When would I receive the $80.00 due to me. [redacted]has not responded to my e-mail. It has been 6 months since my surgery.Desired Settlement: Receive the $80.00 due to me.

Patient / Customer has been contacted and issue has been resolved.
Our Director of Patient Financial Services, [redacted] spoke to the customer and we are processing the $80 refund. It will be mailed on August 1, 2013. [redacted] will follow up with the customer on Monday, August 5th to ensure the refund was received.

It became apparent that my mother , had come down with a bad case of respiratory failure during her operation.
We were initially informed by The Surgical Hospital of Phoenix that this type of operative procedure would require an
extensive amount of physiotherapy .However due to my mother’s condition, we did not receive any such
service, of which would have been Included in our overall cost for the operation at $36,530.00 USD paid in full prior to operation .
I can add that The Surgical Hospital of Phoenix up to this point in time - has refused to acknowledge any of our verbal or written requestsDesired Settlement: With this, I would like to ask that The Surgical Hospital of Phoenix reimburse us for the Physiotherapy treatment portion of operative proceedure as a service NOT rendered
I have applicable documentation as proof of payment ect.
