I am rejecting this response because:first of all I do not have diabetesMy endocrinologist also confirmed that tooBut wanted me to check every six months to monitor itI'm in the pre stages at timesI understand that I came in to see my lab results and the main reason was about my a1c resultsIn this case, if this is a main reason of why I'm seeing you every six months why every six months I have to draw blood to check for other things not related to the visit such as lipid profile, arthritis panel, hepatic function panel, CBC, albumin/crest, urinalysis, etc. Prior to not being diagnosed for pre diabetes I see you annually for a physical check upThis physical check up is exactly the same blood test that is currently been done every six months. I do not need to pay for a $signature as I can get a free reading for glucose, cholesterol , and blood pressure reading now through my work place

Basically the six months check up includes what my wellness form was asking for which was if I had completed my test for a cholesterol , glucose, and blood pressure readingThere was absolutely nothing to fill out bedsides a doctors signature and and address which takes less than minutes to completeSo, I don't really understand that I came for my lab results that included these things that I need for work and was reviews to me at this visitI don't understand why I have to schedule another appointment to review my cholesterol, glucose, and blood pressure results againBasically I'm just going to get the same answers so what is the point
Of scheduling another appointment? I don't want to be charged for a doctors visit bill just to get the same answers that was given on my previous visit and to have to pay another co pay of $in replace of a $fee for a signatureThis is a waste of time as the visit wait can take over an hour wait most of the time and when it's time to see doctor it is less than minutes Please see attached for the form that was needed to be filled out by the doctor.

No I do not have anyLast year I had the same form and when I went in to see dr for the blood draw I presented the form to the receptionist and she informed me that if I needed it signed that day it will be $but if I came back the next time to see my results it will be free of charge to sign the paper but ended up that it was not trueI remembered her name was Happy that told me that but she wasn't working the day I went on to see my results and when the other receptionist mentions it to MrsT** the office manager informed me that she doesn't know what she is doing and that she had to talk to herThat time I paid $to get my formed signedAfter that I talked to my co workers about it and they were telling me that is wrong and I believe so tooTold my family and they think it did t make sense eitherThat's why this time I'm so frustrated over thisDoctor is very nice don't get me wrong it's just ridiculous to pay $for a signature for a test that I've done that was in the lab reportI understand if a test that I needed is not in the results then yes I will have to make another appointment So am I suppose to schedule another appointment to draw blood for the same tests?? I mean we wait for at least or more than an hour in the doctors office who will pay for our time to wait there

*** ***Dear *** *** This is to clarify the response that was sent to you ** ** came to see me on 9/24/for her months diabetes checkup She has been diagnosed with diabetes and is currently seeing an endocrinologist Every months, she would do her blood tests to check on her diabetes status This was the reason for her visit on September ,to see me to go over her diabetes blood tests results It was not a scheduled appointment for her wellness checkup for her work This is why there was a fee assessment for her work physical form which had nothing to do with her diabetes checkup She should had scheduled a separate appointment for her work physical wellness checkup.thanksThomas T**, MDif there are any more questions, please contact me

*** has been under our care since She came to see DrTan on 9/24/for her routine months review of her blood tests.During her check in process, she presented a form to our medical assistant for DrTan to signShe was informed at that time there wi!I be a $service charge for the
formIt was explained to her that any form not related to the main reason of the visit will be assessed a fee If she had scheduled an appointment.to do this wellness checkup and fill out the form, then there will not have been an additional feeHMSA benefits do not include doctor's filling out forms not related to her visit.In terms of work notes, *** has been with us over years, she should have remembered the work notes that were given to her previously without the fee assessment

So, I don't really understand that I came for my lab results that included these 3 things that I need for work and was reviews to me at this visit. I don't understand why I have to schedule another appointment to review my cholesterol, glucose, and blood pressure results again. Basically I'm just going to get the same answers so what is the pointOf scheduling another appointment? I don't want to be charged for a doctors visit bill just to get the same answers that was given on my previous visit and to have to pay another co pay of $20 in replace of a $20 fee for a signature. This is a waste of time as the visit wait can take over an hour wait most of the time and when it's time to see doctor it is less than 5 minutes.  Please see attached for the form that was needed to be filled out by the doctor.

I am rejecting this response because:

I understand that I came in to see my lab results and the main reason was about my a1c results.
