I have reviewed the response made by the business in reference to complaint ID [redacted] and have determined that this does not resolve my complaint.  For your reference, details of the offer I reviewed appear below.

It seems companies can take up to two to three months to respond to complaints filed at the Revdex.com Pittsburgh but if a consumer takes a mere two weeks to respond a case gets closed.
It has taken me two weeks to think about how I want to respond to Three Rivers Dental latest letter and what I would like to do next since the Revdex.com isn't able to take action. 
I am not letting this rest.  Fraud has been committed against me and I want this resolved to my satisfaction. 
First, "New patient free exam and x-rays" that was advertised?  I was billed for and I paid.  How is that free?
Second, "Fees of service, discount, and insurance calculation" that I signed for doesn't mean that the company actually provided what I paid for.  I did sign a financing receipt agreeing to pay for the services...  I did pay for the services... IN FULL but never received what I paid for.  That is what I am so angry about.
Third, if their ledger clearly showed that my insurance had a maximum of $1500, why weren't they billed for said amount?  [redacted] didn't pay out the maximum.
Ironically, Three Rivers Dental hasn't even addressed the fact that I paid for services that I NEVER received.  One crown and two bridges to be exact.  I was "quoted", "billed" and PAID FOR treatment that I never received.  The bridges were never even fitted!
It has been brought to my attention that I can contact my state representative ([redacted]) and the district attorney's office about the consumer fraud Three River's Dental committed against me.  I have also acquired Form 3949-A and filing to the IRS about fraudulent charges and services being charged and never being received in the hopes that the company will be audited by the IRS.  I find it very hard to believe that I am the only one that has been charged for services that I never received.
My initial complaint against Three Rivers Dental will be forwarded to the state representative's office and also the district attorney.

I have reached out to [redacted] in reference to her Revdex.com complaint. I reviewed her ledger with her and alerted her to some facts she may not have been aware of:
[redacted] had specific procedures completed under...

IV sedation. These sedation fees exceeded the original estimate by almost $800. [redacted] will not be billed for these charges but will not be entitled to a refund of what she perceives as an insurance overpayment. Even after applying all insurance monies collected to her account, [redacted] shows a balance of more than $400. We will gladly remove this charge for her but there is no money or charges that we can refund.

 Mrs. [redacted] came into the Jennerstown office as a new patient on October 2nd, 2014 She was diagnosed as needing specific dental treat totaling $8738.00  
We understand that much needed dental treatment can be costly, therefore out of the kindness of our hearts we provided a “New Patient Customer Relations Discount” of  $1085.70. This discount was contingent on the full completion of her treatment.
Given the fact that we discounted the treatment by $1085.70 which would be the cost of the partial denture there is no reason to offer an additional refund for something that was free. 
The $936.95 is a charge that we pay for using a financing company. It is listed on the ledger for accounting purposes. This by no means is a charge to the patient. If the patient would have used cash or a check the treatment amount would have been the same $8738.00 minus any potential discount to the patient. 
Mrs. [redacted] stated that if she would have known of the $936.95 charge, she would have paid cash. The facts are that we never equate any interest charges into the cost of treatment. That is money that we pay the loan companies for enabling an alternate opportunity for a patient to have an affordable monthly payment. Having Mrs. [redacted] pay cash would have been Ideal because we would not have had to pay the $936.95 
 In reality, the original cost for the treatment was 8738.00 minus 1085.70 discounts, minus a charge of 936.95 that we paid, totaling $6716.05. This is approximately 75% of our Fee for Treatment.     
 In closing: I would like opportunity so sit with Mrs. [redacted] and explain these numbers in person. We believe that every patient needs to be aware of the investment in their dental health. Most importantly, would also like to invite Mrs. [redacted] to come in and finish her partial denture. Please keep in mind that providing quality treatment for our apprehensive patients is our goal.

Review: I recently last year approx 6/2015 and had this practice make me bridges for my mouth on the first occasion they loosened and they fixed them for me they were very nice and professional to me on a few more occasions they loosened and they fixed them again for me . One the most recent incident was Appox in March of 2016 they have loosened again I have on many occasions asked them to recement them back in because they keep falling out . The practice in jennerstown pa had offered to fix them for me as recently as last week .the Greensburg office refused to let them fix them .so one March 23/16 I received a letter stating they were terminating my patient /practice relationship for no reason at all . In the letter it states that in the event of a emergency they will see me , so on 4/11/16 they fell out in my sleep and awoken me I almost choked on them which to me is a serious problem I have expressed my concern to the practice and on 4/12/16 I went to the office in Greensburg and kindly asked them to recement them for me in fear I will choke on them at night the manager once again refused to fix them give me an appt. or even give me the owners name and number . There was no reason for this this manager has thrown me out of her office 2 times now for no reason .the work was financed threw lending club in the amount of 3,000 dollars I'm only asking to have them fixed.Desired Settlement: I'm asking of one or the other either refund my card so another dentist can fix them or please repair these ones not askin for much .

Review: I was visiting as a first time patient and I was told as a first time patient X-Ray's are free after visiting on January 12th at 6:00pm not only were they running so late that I didn't get seen until about an hour after my appt and had to sit in the back of the office they had to do an X ray needless to say they did TWO x-ray's only because they messed the first one up by not putting the film in the machine (I was charged for three x-ray's by the way) when only one actual x-ray was done. they also billed my insurance for a cleaning that was never done the doctor barely touched my mouth! also they prescribed a medication to my domestic partner without even seeing her, and when our new dentists office called them they had "no record of us being patients" but they sure didn't mind billing my insurance for things that have not been done. I now have to pay extra to get my wisdom teeth pulled (by the way thank you for referring me to an oral surgeon (must have forgot to do that as well)) because they used some money off of my dental insurance maximum.Desired Settlement: I would like for you to pull the charges for (D0272) BITEWINGS- TWO RADIOGRAPHIC IMAGES for 25.00, (D0330) PANOGRAMIC RADIOGRAPHIC IMAGE 65.00, (D0210) INTRAORAL- COMPLETE SERIES OF RADIOGRAPHIC IMAGES for 90.00, as well as the cleaning that you never did (D1110) PROPHYLAXIS (CLEANING)- ADULT 60.00. I would also like to receive an apology for the ignorance of the staff on that same day towards my family it is wrong how they operate their "business". if the charges cannot be pulled then I would like reimbursed as I have to pay the difference when getting my wisdom teeth pulled Thank You.

Dear [redacted], We are responding to the Revdex.com complaint ID [redacted] have reviewed the patient record from that day. He was seen as a new patient in our office 1/12/2015. He had a comprehensive exam, prophy, and panoramic xray. There was no needed treatment accept for extractions of several impacted wisdom teeth which we do not do in a general practice. He did not have bitewing which are typically done, and were added by mistake so we will refund the insurance company $25- for them. I have enclosed our new patient coupon from our website for “free exam and xrays”. New patients do not pay for them and If a patient with insurance has the x-rays denied by the insurance company due to frequency or any other reason, the patient is NOT charged for them. We do a courtesy insurance adjustment and write them off. He was also prescribed Ibuprophen to address his discomfort issues. We do not prescribe prescriptions for non patients. We do everything possible to keep our patients from waiting but occasionally a we have emergencies that can cause other patients to wait and we apologize for that happening. We do not refer to specialists because that is up to the patients preference and location. Review: Terminated relationship due to poor quality of work. (Started relationship Oct 2, 2014..terminated January15, 2015). I paid in full for all dental work in October prior to start of services. When I terminated my relationship due to poor quality work ( I have photos) I asked for a refund for a partial I paid in full for. Because I had no itemized statement to prove what my $6246.39 covered I Asked for itemized bill for fees charged on this date. Received itemized bill after numerous calls on August 7, 2015. It is evident that the itemized bill was fabricated and I was charged a "provider finance fee adjustment of 15% equaling 936.95 which was never disclosed was added. (for a care credit purchase of $5309 which was paid in full 2 weeks after it was charged.) I am not only due money for my partial that I never received (and is not included in the itemized bill sent me for my payment in full in October 2014), but also the fabricated finance charge of $936.95 which was NEVER disclosed. This dental office received a total of $7746 for 4 crowns and 4 root canals (no partial) This is excessive and I feel as a consumer I was fraudulently overcharged and am entitled to a refund of some sort.Desired Settlement: I would like a refund for the finance charges I found out about in August 2015; refund for the partial I paid for when I paid in full for all dental work (including the partial).

Dear Ms. [redacted] I received the correspondence dated 8/25/15 from you regarding Three Rivers Dental’s response. I would like to rebuttal (in red) the comments in the response with the following: Lack of Communication is the primary deficit with this business. 1. Gil M[redacted] from Three Rivers Dental wrote: “ Mrs. [redacted] came into the Jennerstown office as a new patient on October 2, 2014… (this was a IV sedation case …”no sights …so sounds..everthing done in one visit” kind of experience I expected. As you can see from the prior paperwork, I was still visiting the office for completion of work in January 2015. I had to do all the follow-up calls to communication any plans with progress of my case… ) 2. “Out of the kindness of our hearts we provided a “New Patient Relations Discount” of 1085.70” this was an advertised “kindness” not just extended to me. If the fees weren’t so high this discount would not be necessary. I understand the costs of dental work as I have 2 bridges in my mouth now. It was never communicated this was contingent upon completion.3. Given the fact that we discounted the treatment by $1085.70 which would be the cost of the partial denture there is no reason to offer an additional refund. It’s interesting that I “paid for this” yet it doesn’t appear on my patient ledger of charges. I have no way to know what Three Rivers c harges for a partial denture on the bottom. I have a right to know. Again, no communication.4. “The $936.95 is a charge that we pay for using a financing company. It is listed on the ledger for accounting purposes. “If the patient wouold have used cash or check the treatment amount would have been the same……I have a right to know all costs are incurred for my treatment so that I can make an informed decision to proceed. No communication or disclosure was presented to me. “Having Mrs. [redacted] pay cash would have been ideal because we would not have to pay the $936.95…one sentence Mr. M[redacted] states the cost would remain the same and the next paragraph he indicates it would not have been charged. (contradicts himself). It is convenient that there is no recognition made by Mr. M[redacted] regarding the numerous complaints regarding customer service and quality of work that should be documented by their office. ( Angela (office associate) was my contact. ) The option to have the work completed is a joke. Should a patient who paid for IV sedation still be trying to complete the project months later? After months of horrible communication and misinformation and poor quality work, I decided as a customer, I didn’t trust them with my mouth anymore. I do have that right do I not. I hope that Mr. M[redacted] takes a good look at the trail of documentation that brought me (a patient who has severe issue with even going to the dentist because of fears) to my decision to stop treatment with Three Rivers. I am angered at the commercials…it’s a joke. Two months after sedation, I am still not done. And let me not forget, 3 of the 4 crowned teeth have pain. I had an xray at Hilltop dental which showed inone the crown did not cover the back of the tooth leaving it exposed. I am waiting to see what resolution occurs with Revdex.com and Three Rivers before I proceed further. In closing: I chose to discontinue treatment with Three Rivers because of lack of trust in the facility because of poor quality work and severe lack of communication and miscommunication. I only asked to take the money I spent on the partial to take my business elsewhere. It was when I asked for this that I realized the lack of disclosure of the $936.95. I told the office manager I was going to pay the care credit off that week and that I only used it as a convenience on that day. I was NEVER told if I used cash it would not be charged me. This is a HUGE chunk of money. I have a right to choose and be aware of such a cost saving choice. Keep in mind I paid in full on October 2, and the services started on 10/23. I want the $936.95 and the credited amount on my patient ledger of $173 that they themselves indicate I should be paid. I believe this ledger was fabricated for the sake of “saving face.” If there is no resolution to this through this avenue, I will initiate legal counsel to review the actual charges and seek pain and suffering as some of the work that was done needs fixed. Review: I had a tooth pulled by Three Rivers Dental Group. I never, ever stated that I wanted two teeth pulled but the officer manager MARCUS charged me for getting two teeth pulled without even telling me. When I went in for my appointment the Dentist told me she was going to pull two teeth. I informed her that I never wanted two pulled so she only pulled one. Afterwards I asked the receptionist for half of my money back since I was charged for 2 teeth being pulled and only had one pulled. Even though I paid with cash I was told I had to wait up to 30 days for a refund. After two weeks of nothing I called the office just to check the status of my refund. I was told that Marcus the officer manager was busy and that he would call me back. That was 4 days ago now.Desired Settlement: All I want is my money back. I will never return to this business again because I feel as though I was treated poorly because I do not have dental insurance. To begin with all I wanted was a single tooth pulled and I never, ever mentioned pulling two. When I paid with my cash I was never even told I was being charged for two teeth. I am a single father and every penny counts for me. I informed Marcus of this from the start, so for him to do this to me with my money is really ridiculous.

Mr. [redacted] came to our practice on 5/19/15 as a new patient complaining of tooth pain. We took several x- rays and did a complete exam at no charge to help the patient since he did not have dental insurance. We came to a prescribed tx plan to nerve treat, post core, crown for two teeth and was willing to work on a payment plan to get tx started. Mr. [redacted] came into the office to apply a down payment which was a part of the payment plan process on 5/20/15 and stated that he rather extract the two teeth,due to him being financially strapped. On 5/26/15 Mr. [redacted] came into the office to get the two teeth extracted and before the procedure started told the doctor that he only wanted one tooth pulled cause since we gave him antibiotics they feel better. Mr. [redacted] upon leaving asked to get the money back for the one extraction and since it was six days later told him I would put in a refund request and it could take a maximum of 30 days. I charges him a total of $230.00 for two extractions which was a discounted price cause his extractions would have been surgical and the price would be 230 each, the reason we did this was again to help this young man due to his lack of insurance coverage. On june 16th my office issued a refund of $115.00 to Mr. [redacted] which was under the 30 day refund process that we had discussed, also Mr. [redacted] signed financial arrangement form agreeing to have TWO teeth extracted. Review: I went to this office in Nov 2014 for a consultation. First visit and x-rays are not free as advertised. My insurance was billed for them. After my initial appointment, I received an estimate for my dental work, after insurance, for $2839.00. I was made to pay $600 down to even book the appointment to have the work done. I went in on Nov 20 and had the work done as discussed. I was forced to pay the balance before I was even put in the chair. I charged the remaining $2239.00 to a credit card. Remember, my insurance still has NOT been billed! I had an appointment to come back in Dec 2014 to have my crowns cemented in. The day of my appointment I received a call from a gentleman at the office who notified me that my crowns were not there. They had come in and the dentist was unhappy with them, so they were sent back and I was told that it would not cost me anything. I got charged for upgraded crowns that they said I asked for, which is a complete lie. I never even knew there was a different type of crown. I eventually got there in Jan 2015 and had the crowns cemented in. I was contacted by my insurance company prior to this appointment letting me know that my billing was not being submitted correctly. I asked the receptionist and she said it was taken care of. In Feb 2015 I received the statement from my insurance company and I am being told that I am only responsible for $1353.00, not $2839.00 like I was charged for. My insurance company and the PA dental board are now investigating this business. Three Rivers Dental just keeps saying that I agreed to the amount so therefore I must pay it. I agreed because I trusted them and thought they were honest people! And I needed dental work done. I am now having pain in one of the teeth that was root canaled and crowned. They are very dishonest people and should be avoided at all costs. Review: I was billed for services that I never received. I am receiving bills for an appointment that I never had and when I called to see why I was still being billed having not received their services and they are charging me $1700 for a no show fee. After reviewing what my payments would be I decided that I would not be able to afford it so I never got the work done.Desired Settlement: I would like to have this matter resolved as it is ruining my credit they have had $3500 of credit under my name and I want it taken off of my credit. I can not see how I can be billed for a loan that was never used.

Letter Date: 12/15/2015 Dear [redacted],I am responding to the complaint filed by Mrs. [redacted] on 12/02/2015 Three Rivers Dental completely understands that Mrs. [redacted] has a problem with her maxillary over-denture that needs our attention.Three River’s treatment on Mrs. [redacted] was initiated to replace teeth removed teeth without replacing with full dentures. The over-dentures are retained by the remaining teeth.Mrs. [redacted] originally complained to us that she was unhappy with the fit and her permanent over-dentures. We tried to resolve this issue by adjustments and relining of the appliance. When this approach failed, we fabricated new over-dentures to alleviate these problems. The new over-dentures were inserted on 11/12/2015 including reline/rebase cost ($324.00) at no cost to the patient. Since then we were made aware that Mrs. [redacted] has an issue with the new upper appliance. We have made every effort to schedule the patient to resolve this issue. Review: This office is a money hungry group of people. I went here on nov 4 for a cleaning and check up. they told me that I needed to have all my old fillings replaced so we set an appointment. I arrived 15 minutes before my apointment and was told that they were running behind that day. at 2 oclock (the time of my appointment) I am still sitting. at 3 I got in a chair. they gave me the shots and everyone left the room. good thing I didn't have any problems with the shots. well anyways at about 4:30 half ways done with my work on the left side the dentist went to work on somebody else leaving me sit with my teeth open. I finally left the office that day at 5:30. Now they were suppose to do both sides of my mouth that day, which didn't happen because they were running late. Which I payed for. I have had pain in this work they did now for a month and I am seeing another dentist. I was back to Three Rivers 4 times to get help and they act like I am not in pain. Lets talk about the bill. I had to pay for this work before I even got in the chair. All the work even what I didn't have done. the first visit on 11-4 was to be free cause I was a new patient I come to find out later that this is not true. and they are telling me that I do not get a refund for the teeth that I didn't have done.I hope nobody goes to this office because they are raping insurance companies and the public and could careless if you are in pain.Desired Settlement: I would like a refund to my insurance co and to myself and for this office to be shut down.

Review: My wife was to have all her teeth pulled on May 31, 2013. Due to me having knee surgey on June 3rd, 2013 we decided to cancel this appointment and made a new appointment for July 26, 2013. I come to find out that credit that I needed to pay my bill thru [redacted] Credit was submitted and paid before teh procedure was even done. On July 26th and I got home from work early to take my wife to her appointment and she mentioned that the phone rang and to check for messages before we left. She is disabled and could not answer the phone. There was a message from Three Rivers Dental to call. What I found out was that the anesthesiologist would not perform the procedure to put her to sleep for the prodcedure and that we had to find another dentist. I said fine and asked them about sending the money back to [redacted] credit for the procedure that they did not do. As of this date the money has not been sent and I am being asked for another payment. I have already sent in two payments to them thinking at the time that the procedure would be done. They had talked us into healing dentures but we feel since they would not do the procedure that we do not need to take them. The new dentist that we found said the he needed to remove something else to help with dentures fitting better which they said nothing about and we feel the the healing dentures would not fit. I just need someone to look into this and see why they will not pay the money bank. This will hurt my credit and I feel that I am not responsible .Desired Settlement: For Three Rivers Dental to pay the money they received back to [redacted] Credit and we should not have to take or pay for the healing dentures.

We Had a manager with personal problems that did not address your concern, as well as others. It has caused issues for us. We strive to give all our patients superior patient care. We fell short of that for you . I want you to know, I will be refunding your money the same way it was paid to us, minus the Fee for the set of healing dentures. Healing dentures are made prior to the extraction of the teeth and any alveolectomy that is done along with the surgery. Their only function is to provide you with a smile during your healing process before the permenant dentures are fabricated so you don't have to be seen toothless. Even if you go to another dentist or oral surgeon and have additional surgical procedures done, I can assure you, your dentures that are sitting in our office can be used. A soft liner is added after the extractions to improve the fit until impressions can be taken after the healing process. The cost was $349- each for a total of $698. P lease let us know if you would like us to mail them to you or if you would like to pick them up. I understand your case was cancelled by the Anesthesiologist. He suggested a hospital setting for your own safety, well being And comfort. I am including your receipt for the refund of money paid to us, along with your ledger showing theTransactions. Review: The amount that my dental insurance paid on my claim was more than what the office estimated. I paid their estimate in full and now they will not refund me the difference between what was paid by myself and the dental insurance. I have made multiple phone calls to the dental office with not one return phone call. I have all of my paperwork stating what I paid, what the insurance paid, and what my total bill was.Desired Settlement: I want my money refunded to me. This has been going on since January 2014.

I have reached out to [redacted] in reference to her Revdex.com complaint. I reviewed her ledger with her and alerted her to some facts she may not have been aware of:

[redacted] had specific procedures completed under IV sedation. These sedation fees exceeded the original estimate by almost $800. [redacted] will not be billed for these charges but will not be entitled to a refund of what she perceives as an insurance overpayment. Even after applying all insurance monies collected to her account, [redacted] shows a balance of more than $400. We will gladly remove this charge for her but there is no money or charges that we can refund.

Review: I paid 3200 to have my top teeth pulled and dentures for top. after three tries my dentures are still too big for my mouth.I'm not going back to pay more to have to fix there screw up. They want to charge me for a refitting which should have been covered when I paid in full. Then they sent me a bill for cleaning my bottom teeth that I did not ask for . My teeth are off center and I also have to use alote of fixodent when I use them . I am really tired of having to go back in and its costing me money because I have to leave work to go .I paid for thin dentures and these are not the thin ones . This letter is in regard to complaint #[redacted]. Patient is [redacted], [redacted] presented

to our office for an initial exam and x-rays. He was interested in having extraction of his

upper teeth and a denture placed. His extractions were done on November 8th2013 and a

temporary denture was placed at that time, [redacted] returned for several appointments to

have his permanent dentures made, His permanent denture was delivered to him on

March 21, 2014. [redacted] was told at that visit that if he bad and problems or was in need of

an adjustment to his denture to call us. We did not hear from [redacted] assuming all was well

with his treatment and new denture. In September 2014 [redacted] sent a complaint to our

website that his denture did not fit right. We immediately responded to [redacted], trying to

reach him via telephone leaving him several messages to call us. No response from [redacted],

so we reached out to him by certified mail. We received confirmation that he signed for

the letter. He has never responded to any of our attempts to reach him. He has not

returned to our office to have his denture accessed or adjusted. The certified letter sent to

[redacted] explained to him that the solution to his denture problem may be as simple as an

adjustment to his denture but we needed to See him in order to address any issues lie may

be having with his denture, This visit and/or denture adjustment was offered to [redacted] at no

cost to him. [redacted] never calied or responded in any way to our several attempts to reach

him. Our main concern is the dental health of our patients.

Review: I went in for dental work and was quoted a price for extraction of teeth, temp. denture and a permanent denture. when it was time for my temp. denture, the denture was too big and I could not even put them in my mouth, I voiced my concerns about it and nothing was ever done with them so I was left without teeth that I had to pay for,for over 2 months til my permanent denture was made. I now have the same problem with my permanent denture. I went in for numerous adjustments and I have complained to them that my permanent denture is also too big. I was even overcharged and then told that I wasn't getting a refund because I had to many adjustments. I was never told that I was being charged for adjustments until I called in and asked where my check for overpayment was. I have called in to see if they would make things right on November 12th, and was told that they would get back to me and see what they could do, it is now December 6th and I still have not heard back from them. I spent $2139 and still cant eat or drink anything without using atleast 2 tubes of denture cream a week because of very loose and big fitting dentures. I should not have to pay for adjustments when both dentures where wrong from the start.Desired Settlement: I am very unsatisfied with this place and the lack of help I received from them,I would like for Three Rivers Dental to make things right by refunding me the money it would cost for me to go to a dentist of my choice and have a set of dentures that actually fit my mouth so I can eat and drink somewhat normal again.

Review: First, the New Patient Exam and x-rays were supposed to be free but I was charged $183.

Second, Stacey P was so harried by the other workers, phone, etc. that she miscalculated my 10% by $132.

Third, Stacey P and I talked extensively (Four hours - because of constant interruptions) about what I wanted and settled on 2 partial extractions for broken teeth and 2 bridges for these open areas and four caps. Plus, nerve treatment, post/cores and a deep cleaning, all recommendations by the dentist, Dr. Vickie A[redacted] With this extensive work, I opted for sedation. Total fee: $9017 - $1317 (Insurance estimate) for a total out of pocket fee for me of $6930. We had our sit down and I made payment on May 30th, 2014. My appointment was schedule for June 18th. I was disturbed to discover that by the time my appointment for the work came around Stacey P no longer worked with the office but I was assured that Dr. A[redacted] had heard all of my concerns.

My main concern was having permanent bridges and not the plastic devices (partials) Stacey P had shown me on my initial visit.

Procedures went well except for a tooth I wanted to save. The tooth being hollow couldn't have a cap/crown but I was told that a bridge would not cost extra than a crown, so I gave the go ahead for the extraction/bridge.

I got four crowns... but ZERO bridges. NONE.

What I got was a HARD plastic partial that I was adamant about NOT having and it was only for TWO teeth! Not knowing what else to do, I agreed to try it out for three weeks. When I asked about the other bridge, I was ignored. The partial was horrible. Food immediately wedged between the hard plastic and behind my bottom front teeth. The partial had to be removed and my teeth had to be cleaned before taking another bite but I did give it a test. When I went back to the office, I told Dr. A[redacted] of my displeasure but she just told me that was how it was and she sanded a piece of the hard plastic and reinserted it. This partial (while I am told is the best "NEW" it thing is for cosmetic purposes only, to me. It is not viable to use this device for chewing...biting... or anything else teeth are supposed to do. After being asked to try it again, I told her that it wasn't going to work for me but she was called away and did not return to address my concerns. Partial/Insertion cost $1099.

Fourth, upon renewing/reviewing my dental insurance I discovered that quit a few charges that I paid full price for was covered by my health insurance. Even the anesthesia/sedation used (with my healthcare if more than three extractions or more than three nerve treatments or three of either are performed [redacted] will cover cost of sedation up to 50%... when I reviewed my dental claims I was horrified to discover that Three Rivers Dental only submitted eleven of the twenty two procedures. ($918 of sedation would have been covered by insurance and 80% of $5446 should also have been covered by my insurance = $4359.

Fifth, when I called and asked for my patient ledger, I received it in the mail. NONE of the figures add up to the $9017 I was quoted, even paying the new patient exam and x-rays the total of all charges was $8129 for a $888 difference. PLUS, there was a $301 'debit adjustment' that I NEVER received.

Sixth, there was gum damage by the crown that was placed top front of my mouth... so that smile I paid dearly for is damaged and I was told that it would grow back to look natural. It has been 17 months and they still look the same.

These errors seem to me to be fraudulent. Insurance fraud and customer fraud.Desired Settlement: $132 discount error +

$1099 useless partial +

$301 unreceived debit adjustment +

$888 overcharge +

$918 (insurance should have been charged for 50% sedation)

$1089 (additional of what my insurance would have paid had a claim been made by Three Rivers Dental.

If for any reason the services are denied by the insurance company, the discount will be applied to the patients account reflecting the advertised "new patient free exam and x-rays" as disclosed on our advertising.The Fees of service, discount, and insurance calculation shown on the Federal Truth in Lending Disclosure State was accepted by the patient, as you can clearly see her signature on the appropriate patient signature lines. Ms. [redacted] also signed the care credit (financing) receipt agreeing to pay for the services recommended by the doctor.Ms. [redacted] also stated that she spent four hours with Stacy P. which is ample time for all questions to be answered. Ms. [redacted] also claims that her dental insurance: [redacted] would have paid more than what they did, but the maximum allowed by her dental insurance per year was $1500.00. The ledger shows that $1500 was covered, and dental insurance NEVER pays more than the maximum allowed per year. Ms. [redacted] also mentioned that her medical insurance would have picked up the IV sedation, but as Three Rivers Dental Group Is a Dental practice we do not have the means to bill medical insurance.
