Good Morning, Please be advised that upon researching this patient’s account, we have confirmed that the patient was never seen at the office on 9/3/15. They misunderstood the statement that they received. The actual date of service that the
patient is being billed for is 7/28/15. The patient’s balance was applied to their deductible. In error, they viewed 9/3/as a date of service, when it was actually the date an adjustment was made to the account. As a courtesy to the patient, we have cleared the balance due and sent the patient a statement noting their zero balance. Please advise if there are any additional questions pertaining to this account. Thank you, Lisa
