NATURE OF COMPLAINT: *** called writer to complain that he was very disappointed in Parkview Hospice
DESCRIPTION OF COMPLAINTS: He noted that he and his grandmother were estranged and he had given instruction that he would not be coming
to the ECF to see his grandmother but that she was not to be informed of that informationSocial worker *** *** was not aware of that instruction and during the course of conversation with the patient brought that up which upset the patientPer ***'s request *** was replaced on the case by *** *** LSWHe advised that *** did not contact him as he felt she should and he also was upset that he did not know his grandmother was declining and felt that Parkview Hospice did not communicate with him

ACTION: Investigated the actions taken by Parkview staff using interview with staff by *** *** Hospice Supervisor Huntington.\/Whitley branch and documentation in the patient recordInvestigation shows that *** *** the newest social worker scheduled in the patient home tried several times to contact *** and they played phone tag (confirmed by ***) and that *** left a message that he had no needs at that timeDocumentation in the nursing note identifies that *** was notified by the ECF nurse at the time hospice nurse was in the facility, of the patient decline on September Hospice nurse phone number offered for a call back and he declined saying he had her number and he did not contact the hospice nurseIn addition *** tried to contact him and left a message on the 16th and *** did not return the phone call
RESOLUTION: Writer spoke with *** today re: results of investigation*** states that he remains disappointed in Parkview Hospice and maintains that he did not receive any messages from hospiceWriter advised him of the Patient Advocate program at Parkview and how to contact themHe verbalized understanding
