Dear Ms***,It is unfortunate that Ms*** is dissatisfied with the services she receivedHowever, contrary to her claims, we believe the treatment recommendations and subsequent care were within the clinical standard of careLikewise, the financial aspects regarding Ms***’s treatment were appropriate. For the sake of simplicity this correspondence will address each of Ms***’s core concerns in turn.Specific to Ms***’s contention she was billed for services not rendered, Ms***’s chart record documentation will support the contrarySpecifically, treatment rendered to date includes: a root canal on tooth number 3, the extraction of tooth numbers and 29, preparation of tooth numbers and for crowns (including fabrication and placement of temporary crowns), as well as commencing fabrication of upper and lower dentures. It appears Ms*** may be under the impression that because she has not returned to complete the crowns and dentures she elected to start, these services are considered non- renderedHowever, the majority of the work has in fact been completedNonetheless, payment is expected at the commencement of such treatmentMs***’s insurance carrier supported both of these positions when she filed a grievance earlier this yearExplicitly, her carrier, after review of all clinical and financial documents, informed Ms*** she was charged in accordance with her benefits and should return to the office to seat her permanent crowns and dentures. From a financial perspective, the Financial Arrangements, Informed Consents, and all charge slip(s) support that Ms*** had agreed and consented to all recommended treatment, both rendered and or services planned for a future dateMs*** executed each of these forms after in depth conversations and her full understanding of the same.*** *** *** *** *** ** *** Page Revdex.com RE: Ms*** ***Lastly, the use of electronic patient records is fast becoming the standard due to regulatory requirementsMoreover, electronic signature pads are utilized throughout society and something most are very familiar withIt is our practice to display the form for which the signature is being affixed on the computer screen for reviewOf course, this is after the form has been explained to the patientIn addition, our computer system affixes the signature to only the open documentA single signature cannot be affixed to multiple forms. In summary, it is unfortunate Ms*** is dissatisfied with the services she received; however, all treatment rendered were within the clinical standard of careAdditionally, it is our protocol to discuss treatment options with each patient and obtain specific documentation of the same once the patient indicates that he/she is fully prepared to accept a course of treatmentAs we have done with Ms*** and charged her accurately for treatment rendered. However, please know that in an effort to resolve Ms***’s concerns and in the spirit of good will, we have refunded Ms***’s source of payment in fullAs such, we believe this matter to be resolved and ask that the Revdex.com consider the same. Please let us know if you have any questions.Respectfully, Tierrasanta Dental Group*** *** *** *** *** ** ***

Revdex.com:
I have reviewed the response made by the business in reference to complaint ID [redacted], and find that this resolution would be satisfactory to me.  I will wait for the business to perform this action and, if it does, will consider this complaint resolved.Thank you.
Regards,
[redacted]

The issue is that I did not receive a copy of the receipt, contract or treatment plan. When I feverishly asked for clarification I was then given the run around. No answers & I was ignored. Then I notified them that I do not want their services any longer, as I don't have any dental codes for...

DeltaUSA or paperwork to review, they were not helpful. So, the issue is that I cancelled services with them by the 25th of July 2015. Not that I am unhappy with dental services as much as, I'm unhappy with benefit coordinator lack of information, no documentation or explanation at time of signing digital box. Telling me to talk to Dr [redacted] about it, yet he was not available or in office for  help & they blew me off!How can they hold me to a contract that I was not aware of until it became due?. Even then, I only received a treatment plan a month later with my electronic signature on a document I saw for the first time! Now over 6 months later I receive a signed receipt from them obtained by the California Attorney General office that I see for the 1st time. I've had several dental appts at other Delta Dental group dentists as a result of Tierra[redacted] Dental violation of Patients Rights and no review or explanation of treatment plan. Their answer: "YOU signed it!" Then why didn't I get a copy of it before August 25, 2015, & why did they bill the Commenity Bank Smile Generation Financial w/o my knowledge, after I cancelled. Because they believed they could get away with it by ignoring me because of my mental disability & their Greed!Thank you[redacted]

Review: Unconscionable Business Practices to include 1st visit office staff requesting signature on an electronic signature box, while I am reclined in a dental chair during dental procedure. Then, ignoring All inquiries for a copy of whatever I signed. No one available for explanation of services. And, the final blow--- They received a $2000 advance payment for services not rendered from their own [redacted], in my misspelled name & w/o my knowledge.

I'd declined to continue services w/them by phone, and patient email survey, they ignored me & I had no paperwork & month or so later....so I filed grievance w/Delta dental who says dentist requests I return for services. I call the dental office, reminding staff that I never even received a Treatment Plan nor did I receive a contract; and furthermore the account had been closed and I don't want to continue services with them.

I was told "YOU SIGNED IT". Next day mail, I received Treatment Plan dated with1st visit date, and printed that day; with digital signature.

I sent certified letter requesting refund to be sent to C[redacted] because I am harassed up to 20 calls each day by debt collection and my credit score is now negatively affected.

It is against the law to enter into agreement w/o parties involved being aware of contract, I believed. And, not giving me a copy until $2504 due?

Additionally, my anterior bridge (not on their treatment plan) broke off after extractions from Tierrasanta Dental & I had cementation charges from another dentist on the network (Grove Dental in Lemon Grove) who also wanted me to pay extra for services fully covered by DeltaUSA.

I am a U.S. Veteran of the Navy. I am 100% Disabled. And I am on fixed income. I'm appalled at their unfair undue pressure & services fraudulent contract practices to take money from senior citizens.

I filed complaints w/AttyGen & FTC.

Thank youDesired Settlement: Billing adjustments

Refund

Correct credit report negatives

Stop the debt collection harassment calls
