We did not just take one (1) sample. We took a total of seven (7). This assignment has been open for well over days. Yes ms*** has not received her report. TITAN Environmental Solutions, Inccannot release any report unless payment is secure and received. At this point, if Ms*** would like to settle for $315, TITAN Environmental Solutions, Incwould settle the amount of $315. Attached is the credit card form for Ms*** to fill out and submit back to us
Once TITAN Environmental Solutions, Increceives payment, we will send out the limited asbestos survey report
We do hope to resolve this issue

Regards,

The service were never rendered nor was a call made from Titan regarding the payment not being processed. I made several attempts to contact [redacted] in accounting and left voicemails with no return call. I had no choice but to have services conducted else where.

Today I had a call from Titan from a man name [redacted] who left his ####-###-#### ex. [redacted] that I will give him a call tomorrow. However,  I am asking for the 7 sample taken to be identified in writing as this was not the agreement. I have had the data retrieved from my computer and will be picking up tomorrow to try to retrieve the initial agreement as the documentation that were sent to me by Titan's collection company were altered after being signed. This is considered to be fraud.

I will forward it to you once received. Please advise if this has to be mailed as I have not received a response that you are in receipt of the documentation I sent you via email attachment. I believe this misunderstanding to be between the serviced agreed to with [redacted] and the tech along with her leaving the company to confirm the agreed services. It is unfortunate but we should not have to pay for service not rendered and be put in to collections for the lack of customer service follow up by Titan.
My mother has stage 4 lung cancer and is an elderly woman. This has been very upsetting to her as we have never not paid for a service rendered. With that being said we should not have to pay for a service never rendered. Once again the credit card information was given to the tech and it is not our fault that the 3 digit # was not taken down by the tech. Please advise what right Titan has to try to collect on a service not rendered when no calls or letters were ever sent or made to us in almost a year of inspection?
I called the collection company and Titan in efforts to resolve this issues and in every case was talked to in a very deeming way with no explanation to how this was sent to collections or how the amount was calculated. I have to date only received alter documentation and different amounts presented to me on the due amount. I have had to take time away from work to send letters and file complaints. I don't feel any obligation to pay for service not rendered. Nor would I send payment to company altering documentation after being signed.

Review: I had agreed to a limited asbestos survey performance with a survey report to be included. Credit card was never billed and survey never received. I tried several time to reach the women my contractor at the time worked out all the details of the service and amount to be billed to my account with no return On 5/7/13 I received a past due bill with the wrong billing amount. I now called the office and spoke to some man who looked up my account information and told me that they account woman I was dealing with was no longer with the company and that the tech who performed the service never got the 3 digit # on the back of my card and since they could not charge my account the survey report was never sent. I explained to him that I was never contacted and that I left several messages for the accounting lady with no call back. I explain to him that the billing amount was incorrect and that the survey report was never received. I asked him to have a manager, owner or someone in accounting call me. I never received a call back. On 4/29/14 almost a year later I received a call and an email stating I was 357 day past due. I sent an email explained once again that services were never rendered, the billing amount was incorrect and no one called to inform me of the issue with the credit card right after the inspection nor did anyone returned my call. I once again asked to speak to a manager or owner. I saw that a Tony was copied on the email and called him the next day. He was rude and didn't want to investigate only wanted me to give him my credit card information. I finally had to hang up the phone as I was at work and all he was doing was trying to force me into paying for a service never completed.

I need to edit my claim as I was able to confirm the correct information from the Contractor I was working with until now. He confirmed he had spoken to [redacted] in the accounting department and she had given us a price for the services for $250.00 for a limited asbestos survey that was to include the survey report all inclusive. His name is [redacted] and his cell# is ###-###-####. The inspector that came out from Titan can also confirm this information.

The service was performed on 5/7/13. I received a letter for past due payment in December of 2013. Which I called on tried to address with no success. The bill indicates PLM bulk samples (7) @ a rate of $15.00 and for (1) limited survey@ a rate of $250.00 (1) survey report @ a rate of $50.00. The inspector had to call [redacted] in accounting for us to confirm the agreed upon the amount due. The paperwork I signed had no amount due on it and as I was concerned about this the inspector took a picture with his cell phone and emailed it to me.

I have since had the insulation replaced in the attic and was told that the heater pipe does have asbestos. The inspector never went into the attic but only open the attic door and took a sample of the blown in paper insulation. Samples were given to him of the popcorn ceiling that the contractor had collected which he said he could use. I do not remember 7 samples being taken.

The reason for concern was that I was doing remodeling on my mother house and she had just been diagnosed with stage 4 lung cancer. I have all documents I can fax to you if needed. I have been very upset with all of this and trying to care for my mother. It's not about the money but about the services not being rendered and the company not willing to work with me. I am very busy with work and trying to care for my mother. The company has now sent this to collection and my mother has been very stressed as we have never just not paid a debt.

Last week my mother received another collection notice which included a copy of the paper I filled out at the time of the inspection however this paper has now been altered to reflect an amount due that was written in after I signed it. I will send you a copy once I pick it up from my mother.Desired Settlement: The job was never finished as the survey report was never sent and the billing was incorrect. I had no way of resolivng issue this with the company. I had to move forward with other services due to the companies lack of follow up and delivery of services.

Review: In May 2,my flooring in hallway got wet and my wood floor got affected.I called the State Farm Insurance to report the incidence. They made me called a plumber,of which they cant detect where the leak was coming from. This courtesy plumber, made the decision to call the Shamrock company.about 3 people came over checked the flooring, then all of a sudden here comes another person from Titan company which I did not call or hire.So they were several people in my house that I did not call.I was having a 102 fever, cancelled my cruise in Alaska and tried hard to figure out the situation. After the Titan cut a piece of wood from my floor, the guy made up a form and said "this will be claimed and paid by State Farm"

showed me where to sign.He did not explain to me in detail. He just told me that this is a proof that they were the one who did the work. The shamrock made me sign too. This ugly

State Farm offered to send me American Detection Company free of charge. Why then they rushed me to call a plumber of which I paid $ 275.00 ? I was very very upset. Then I found from insurance that the damage was not covered. After a month I received a bill from Shamrock $500.00.I called the manager explained to him that I did not hire any one of them.He easily cancelled the bill.BUT this Titan sent me bill of $575.00 I explained but would not listen. However I paid $225 and I don't want to pay anymore. They sent me a FINAL DEMAND

letter full of intimidation like legal action, paying attorneys fees etc. They took advantage of me.I was very sick.I asked the manager who called them to come to my house?I only called a plumber, paid his service even if he did not resolve the problem.Everyone appeared to my house. The titan said they checked if I have mold in my house. It came out negative.

I will file a separate complain to State Farm why they did not send me a free charge service they have for the customer, and let me call a plumber that charged me $275.00.This happens in May 2 2014 My point is solve problemDesired Settlement: Refund or billing adjustment. I am a senior citizen 68 yrs.old was so sick at that time with 102 fever. The company has to explain in details the document they are presenting to customer especially its in fine print. They also must have the customer fill up the form so I can have the opportunity to look at it closely at the same time needs to explain what is in it,and not just asking me to sign.

Review: On 1-24-2014 Titan performed an environmental assessment service of my home. I never received the hazard evaluation report from them. I was asked by [redacted] the person conducting the inspection to sign a document indicating he did the inspection when in fact the document was an authorization to perform work for the amount of $495.00. I did not callTitan to come and do an inspection, it was the plumbing co. drain rite whoordered them to call me and set an appointment. I did comply with them to do an inspection. I was informed by drain rite (Brandon) that it was required by law. So, my complaint is why do I have to pay Titan when in fact the results for their inspection was never delivered to me but drain rite in an untimely manner. Report was produced after work was finished.Desired Settlement: Not be paid for services rendered untimely. Work began on 1-24-2014 and finished 1-26-2014. The results from the inspection were not delivered to drain rite until 1-27-2014. Drain Rite came and picked up their unused equipment on 1-27-2014 early that day.

Response:

I received this complaint for Mr. [redacted]. In response to Mr. [redacted] response, TITAN environmental did perform an asbestos and lead assessment at the property. We were referred to Mr. [redacted] by a plumbing company to call to see if he like to proceed with asbestos and lead testing. Mr. [redacted] had a water damage and the plumbing company referred us to Mr. [redacted] to see if he would use our services to test for asbestos/lead at his property. We contacted Mr. [redacted] and contracted the asbestos and lead testing with him.

The analytical results were given to the plumbing company which relayed the results to Mr. [redacted].

From our understanding, Mr. [redacted] insurance company is not covering our services. On our work authorization contract with Mr. [redacted], it states it is fully understood that client and its agents, successors, assigns and heirs are personally responsible for any charges not covered by their insurance company.

On 2/26/14, I spoke to Mr. [redacted] to try to resolve this issue and he let me know he did receive our results but not from us. I explained it to him we sent it to the plumbing company and they had let him know the results so they can proceed with repairing the property as soon as they were notified. Mr. [redacted], did let me know the asbestos was found and an asbestos removal came to assist him.

As of 2/26/14, Mr. [redacted] still owe TITAN Environmental for $495.

Please give me a call if you have any questions.

Enclosed: The open invoice, the analytical results, and the signed contract by Mr. [redacted].

Have a Good Day!
