Complaint: [redacted] I am rejecting this response because: In response to what Doctor [redacted] statedI was NOT in pain when I walked in to the office, I told every single staff member that and I have a witness proving thatI did NOT asked for my tooth to be pulled, again I told every single staff member I talked to and also have a witness proving thatI did NOT give consent for the first round of anesthetic I only gave consent to pull the tooth after they shot me up, again have a witnessAnd there was multiple phone calls before I told them I wanted a refund and if I didn't get it I was going to report them to the Revdex.com and Texas dental board Regards, [redacted]
Complaint: [redacted] I am rejecting this response because: I signed consent to extract the tooth after I was drugged upI DID NOT give consent to be drugged upWhen I asked why I needed to be I was told "you need it" and that can be verified by my witnessAnd when it came to signing consent about extracting the tooth the staff member (name can not remember at this point) said "well you're already numbed might as well go ahead and do it" that also can be verified by my witnessAlso cleaning was never done that I paid for Regards, [redacted]
