I submitted Aflac dental claim forms to the location manager (***) in mid November 2019. I requested that the forms be filled out for any treatment that was done on me for the years of 2016- through current which would have been 2016 through 2019. I followed up two weeks after I submitted the forms because that is the timeframe I was given by the location manager, but upon my follow up call I was informed that the forms were sent out to their billing department and were not complete. I continuously followed up just to be told many reasons why the forms were not complete. At the end of the back and forth the forms were never sent out to another office they were with the billing personnel in the same location the forms were dropped off at which is my doctor's location. I need to continue my dental treatment but cannot attend my appointments if I still have not received the previous dental claim forms. There are deadlines I have in order to be able to submit these forms and it is not okay that I am paying Aflac for supplemental benefits but cannot receive my benefits due to my primary dentist office not doing their part by simply filling out the forms. I was understanding to the location that I was asking for a few years of treatment but it is now about to hit 3 months with no response!

Finish the job I would very much appreciate my claim forms to be filled out on the Aflac dental claim forms that I dropped off to the location. This dentist has been very good with the doctor's they provide and I would hate to have to go elsewhere due to simple business operations that are not being met.
To whom it may concern,
Patient *** brought the forms which were mailed back to the patient as they were not addressed for our location to fill out/complete, as the forms have a different treating provider and location. In turn to try and help patient submit her requested claims, the office provided by mail it's own claim forms to help facilitate patient in submitting her claims. Claims to Aflac Supplemental are to be submitted by the policy holder not the office. We thank you for your understanding.
