I was a patient at touchdown interventional pain Mgmt where my first visit was on 5/24/18 I had scheduled the visit weeks ahead of time and when I arrived in the office I was told that I would not be able to be seen unless I paid $400 upfront as it was my deductible. I paid set amount as well as $36.80 on 6/11/18 & 6/20/18 and on 8/27/18 I paid $165.60. In 9/18 I obtained Oregon health plan (OHP)insurance who backdated my insurance to cover my visits all the back to 5/18. I notified and gave a copy of my new insurance card to the office biller Monica and their third-party billing company and asked them to please bill OHP for prior visits that I had to pay out of pocket. I again notified Monica and the third party biller on 10/17/18 after I had received a bill. Monica assured me that the issue would be fixed and that I would be reimbursed shortly. This had continued on until around 4/19 when Monica was no longer able to handle the situation as she kept stating that it was their third party billing department that continued to make the errors and that her manager Rene was dealing with them. At that point I was handed off to a Whitney who was also an office biller, Whitney and I communicated weekly in the beginning as she tried to work out issues with the third-party billing all the while communicating with her office manager Rene. On 10/22/19 I spoke with Whitney again trying to find out why the account had not been refunded and questioned whether I needed to file a lawsuit or what. The next day on 10/23/19 I received a call from Whitney stating that she had contacted OHP and that they were no longer able to bill for the 5/24, 6/11 and 6/20/18 visits as it was to far back for OHP to cover. At that point Whitney stated that I would not be refunded the money and that they could only refund me the $165.60 for my 08/27/18 visit as they were still able to bill for that one. In response to the claim from Mr., I would like to say that I have been working with our billing team to resolve theclaim that Mr. charging that he is due back funds.

Mr. first visit to this office was 5/24/18, at that time he provided us with his commercial insurance card from ProvidenceHealth, we collected what the insurance company said would be his deductable of $400.00.

On his next visits 6/11/18 and 6/20/18 we collected $36.80 for each, per Providence. On the next visit 8/27/18 he in fanned usthat he no longer had Providence insurance and wanted to pay our office visit cash price which was $ 184.00, because he paidon the day of his visit we gave him a discount of $ 18.40, he then paid $165.60.

The total Mr. paid to us was $639.20.

We were notified in September of 2018 that he had OHP, (he has not given us an insurance card from OHP), but we were notaware that they had backdated the coverage to May of 2018 until much later, making it only possible to go back and bill anyother visits but the 8/27/18 visit, we have refunded Mr. the $ 165.60 he paid, please see the cleared check included.

Whitney spoke to a representative of OHP; the representative informed her that because we were not given the informationabout the back dating, that we were right in keeping the amount collect for visits prior to 8/27/18 due to timely filing.

Mr. was discharged from our office in June of this month, so he is no longer a patient of this clinic, and we would likefor his harassment of our employee's to stop so we will refund the amount of$473.60 that he is asking for, even though we arecompletely in our rights to not refund him.
