In regard to ID 10251703, we have determined that the issue with Mr [redacted] is potentially to complicated to resolve without legal assistancel have included Mr [redacted] s signed and dated release of liability (the items attached in the photocopy are the sterilization record for the equipment used for Mr [redacted] s tattoo, it cannot be removed from his paperwork so | have included a blank form for your review) along with photos of pertinent items that will help resolve Mr [redacted] s concernsI’ve numbered all of Mr [redacted] s concerns and then noted the line of the release of liability that will clarity themThe signed release form covers all of his concerns except the deposit, which has been sent via registered mail and once received I will send you a copy of the receipt proving that he did receive itI think after you review the information you should be satisfied that we have acknowledged all of Mr [redacted] s concerns and clarified all of Mr [redacted] s issuesThe only outstanding issue would be the return of his deposit which we have sent to him twice ln years of business I have never had any dealings with the Revdex.com before but I do appreciate your assistance in this matterIt the following doesn’t explain the facts to Mr Charkian then the issue is to complicated to resolve without the legal assistance of our attorney Mr [redacted] lf you wish, l can place the Revdex.com in contact with Mr [redacted] lf this becomes the situationAgain I appreciate your help in this matter Sincerely SMM [redacted]

RE Complaint: 10251703I am rejecting this response because: As my wife was also in the room as an eye witness when the tattoo session began and ended, 1.) Plaintiff paid a deposit that was never returned and was left with a permanant tattooing that is very unprofessional
2.) Although, Traditional Tattoo did not finish the tattoo I paid for, the area worked on should have been to cleaned and sanitized, even if the tattoo gun was running for only five minutes the lack of cleaning it afterward caused a small infectionAs a consumer I should be compensated by having my tattoo fixed or paid to have it fixed professionallyI should be compensated beyond my initial $deposit as I may likely have to have it covered up. 3.) I showed up minutes early to said appointment being how I wanted to go over the specifics of the art images I sent to Ms*** via Facebook a little over a month before the shceduled tattoo session; my wife and I were both surprised how Ms*** did not want to really do the tattoo where I wanted it, she kept insisting to get it done elsewhere and to do it largerI allowed her to "make it larger" to some degree, however to "keep it where I wanted it" as a "compromise" after some final specifications on color scheme and shading Ms*** printed out the pattern outline and then laid the outline on my skin where I wanted it, then excused herself for a cigaret break. After her break she came back in and began to work on me and after five minutes she muttered "I better get the picture to see what it looks like." She did not have a printed copy of the StMichael vs Lucifier tattoo I was going to get in hand to reference, so she left the roomIt was at this point after another two minutes that said co-worker *** *** came in the room to inform us that Ms*** felt sick and that he would be leaving to take her to the emergency room. Upon Mr*** walking out of the room, my wife did follow him and request to have my arm cleaned up and wrappedMr*** came back with tape and plastic wrap and so I asked him "what's going on? Is she okay? Is there anyone else who can finish my tattoo? Mr*** replied that "He wasnt sure what's going on." I asked "can I reschedule or something" he said to leave my number." "that he wasn't sure." and left the roomAlthough he did cover my arm with plastic and tape I failed to receive any proper cleaning and/or sterilizing treatment causing the infection noted on the submited picture, also I was never given any information on aftercare treatment for my tattoo as I beleive is required by health and safety measures.It should also be noted that, Pliantiff after two days of waiting to see if he heard back from Ms*** (or anyone from Traditional Tattoo) to reschedule, called the shop to find out how to get this embarrassing tattoo finished or fixedIt should be noted that the tattoo Ms*** left on plaintiff's arm began to get a minor infection by this timeWhen I made this call I immediately asked if a manager was available and requested to speak to oneThe person who answered this call said that "No manager was available to help me at this time." I asked when Jilian *** would be back in and he said that "she was scheduled to work the next day." Once again I left my phone number in the hopes of hearing back from Traditional Tattoo and resolving the issue, I mentioned that my tattoo session was interrupted due to what I was told was an emergency, and that I wanted to find out when I can reschedule
The following afternoon I called Traditional Tattoo in order to speak to Jilian *** or someone in a mangement position but once again I was not able to get anyone on the phone. Seeking a resolution or some clearification to what is going on Plaintiff's wife Lizbeth Charkhian called the shop and was able to briefly speak with Ms*** and was told that "Ms. ***'s boss took care of it and will be talking to your husband." Around this same time Plaintiff received a phone call from Shad Perlich the owner of the shop who informed him that Jilian *** or any one else in that shop was not comfortable finishing my tattoo due to me showing insensitivity and for being rudeI disagreed with his decision and said that "he was being unreasonable" and explained I never made rude comments and that he should realize that I had "the right to call to get my tattoo issue resolved." Shad Perlich has taken this a step further now that he himself is saying that I made inconsiderate remarks and comments, this is inexcuable conduct and is very unprofessional and on its face is a bold lieI do work out of an office that has all phone calls recorded and saved and this was the phone line that he called when he "Practiced his right ro refuse service" phone call it should be noted that this one phone call is the only contact I ever had with Shad Perlich
In closing Plaintiff *** ** *** rejects Traditional Tattoo Inc.'s response since it was written in a way as to try to dodge accountability, and erroneously exaggerated Plaintiff's follow up phone calls when he tried to reschedule an appointmentFinally, the permanent tattoo on his arm has yet to be fixed and a refund has yet to be issued and the company can clearly use a reminder on acceptable Health and Safety standards

Review: I had waited for two months for an appointment for a tattoo with Jilian Welfard ,I spent $50 as a hold deposit. I showed up to my appointment 30 mins early to go over the tattoo size with the artist. She finally sat me down and began to shave my arm and then laid the stencil down and began to tattoo my arm. After 5 mins she excused herself and sent an other employee in to tell me she couldnt finish due to being sick. I was then left to excuse myself without being told about when my tattoo will be finsihed/ worked on again or even to be sanitized before I walked away. I was left with a very bad tattoo and it started to get a slight infected due to being not being cleaned and covered or given instruction. I called two days later to find out how the tattoo artist condition was and to find out when I will be going back in to finish. I was surprized to find out that Traditional Tattoo now saying that they do not want to help finish my tattoo for being insensitive to their artist. I now have smothered ink on my arm and a slight infection.Desired Settlement: I would like Traditional Tattoo to fix/ finish the tattoo or to pay to have it fixed or redone.
