I spoke to *** on June 25th when she called to ask about billingShe was unsure of who she spoke with to cancel her billing and had no record of having done soIn speaking for a few minutes I suspected that the employee *** said she
spoke with was *** However, because *** was not sure I could not be sure*** signed an agreement outlining the terms of the billing (attached) it clearly states that the tanning packed continues on a monthly basis after the initial term ( in places on that agreement) and any cancellation needs to be completed through written certified mail within days of billingI asked for the cancellation and she did not have any recordsI am not unreasonable so even though it clearly states no refunds on the agreement (where she initialed) and on the release form she signed I did give her a credit on her account at the store to be used as any timeI also spoke with each of my employees to see if they could recall speaking with her about a cancellationNone of the employees can recall that conversationUnfortunately there is no record of cancellation on either side there for the billing went throughIt has since been cancelled (since I was notified) and a credit placed on her account although it is against policy and on all forms she signedForms attached: membership agreement see paragraph and numbers and Release form please see number and paragraph on page
