response to a complaint made by patient *** ***, who received dental treatment at our Silhavy Creek Family Dental practice in Valparaiso, INThe patient was seen on 9/9/and brought with him a $coupon good for an exam, xrays, which includes a panoramic and a full mouth series, and a healthy mouth cleaningThe cleaning portion of the coupon is valid only if oral health qualifies (exhibit #5)That statement is printed clearly on the couponDuring the patient's examination, he was diagnosed with periodontal disease by our hygienist and dentistTreatment for periodontal disease requires a different type of cleaning called Scaling and Root Planing, which is much more extensive and goes below the gum line in order to clean the roots of the teethLocal anesthesia is generally requiredDue to the patient's active periodontal disease, his oral heath does not qualify for the cleaning portion of the couponI have included a copy of the patient's periodontal probing chartNumbers of mm or greater indicate periodontal disease (Exhibit #3)

My name is [redacted] and I am the Regional Manager Assistant at Transitions Dental Company.  I am writing in response to a complaint made by [redacted], who is a patient at our office, Glenbrook Family Dentistry.  We have no record of any prior attempts to contact us regarding...

this complaint. 
This patient's first visit to Glenbrook Family Dentistry was 11/20/2013 when she presented at our office with sensitivity and severe recession on tooth #3.  Our doctor and staff placed a lingual filling in that area to hlep with the sensitivity.  It is noted in the patients chart that she was advised that if the tooth becomes severely sensitive, throbs, or aches then additional treatment may be required.  I have included the chart notes, exhibit #1.  The patient paid $163.00 at that time.
The patient was treated on 12/16/2013 and 2/21/2014 at no charge because the filling came out both times.  She was advised both times that additonal treatment may be required if the filling doesn't hold or if sensitvity persists.  The patient was also advised that if the filling came out again, a larger area of the tooth would need to be prepped in order for the filling to stay in place and that additonal charges would apply in that case.  Please see the chart notes that I have included, exhibit #2.
On 5/28/2014, the patient called Glenbrook Family Dentistry stating that her filling came out again.  Our office assistant, [redacted], spoke to the patient and explainted that we could see her again, but there would be a fee this time.  The patient became very irate and verbally abusive, threatening to come to the office and cause a scene.
Unfortunately, it is not reasonable to refund the patients money.  She came to Glenbrook Family Dentistry in November 2013 with an existing problem of sensitivity under a crown that we did not do.  We have also seen her two additional times since then at no charge.  The patient was advised each time she came that additonal treatment may be required to alleviate her problem.
