The facts are thisIf TMSC alleges this is a rental, we would dearly love to find how we were ever made aware of thisWhere is anything documenting an agreement? Where are the terms? TMSC has afforded an altered piece of paper titled "Delivery Ticket" (marked copy #2) with the small printed word "rental" circledYou will note that our copy (marked copy #1) of same Delivery Ticket does not have that word circledWho did this and when only God knowsAlso, the bottom of our copy of this document says "Final Total = $540.20" and their entire copy has all kinds of changes, alterations and the bottom portion has been superimposed to show a signature that does not show on our copyIt is obvious that this has been altered because the heavy line at the bottom is crooked on their copy and straight on oursThe entire packet afforded to you by TMSC is full of misinformation, untruths and lies as I just explained about the Delivery TicketI might point out one more particular discrepancyOn page titled "Plan of Service and Customer Instructions" (marked copy #5), Ms***'s copy, it says "signed by client," suggesting this is a signatureHowever, we never signed nor printed a name on this page as per our copy (marked copy #6) and we are wondering who did and why Ms [redacted] implies we did? We have indeed and m fact contacted legal counsel in this matter and he said that any further communication from TMSC or Merchants and [redacted] making any claims or demands, should be trashedMs [redacted] , I have wasted over hours of my time, and the same applies to my wife, dealing with this obvious fiasco perpetrated by TMSCWe are including a copy (marked copy #3) of a TMSC ad online for the same wheelchair that we received, stripped free for $the same chair that they are a for $from usAnother misstatement, or black lie told by Ms [redacted] is that the prescription for this wheelchair was dated 4/1/You will note that our copy (marked copy #4) of the actual prescription shows 4/3/How could the delivery ticket have been made before the prescription had been written? In addition, the info at the bottom of that page shows that [redacted] checked "lifetime" as length of needWho would be ignorant enough to rent anything for a lifetime? After we received a demand from TMSC demanding a final payment of 899.30, I contacted them and spoke to a person by the name of [redacted] who said he was in "accounts." I told him I wanted all the records they had on this wheelchair transactionI spoke to him on this and a later occasion and explained how we would resolve the matter, but [redacted] would not agree with calling the matter settled as isHe still demanded $I have my two conversations recorded for my protectionMs [redacted] stated that they had spoken with us several times in 2014-explaining "blah, blah, blah" another black lieOur only contact from TMSC after they delivered the wheelchair was excess demands for paymentMs [redacted] , we appreciate your effort and time in this matterWe know from our own experience in this how much time is consumed, but there is no alternative but to look at the facts as they areWe have learned from this how much fraud is going on in the medical supply field especially with senior citizensWhen we googled "medical fraud on durable medical equipment" we found page after page of complaints and descriptionsWe can only comment to Ms [redacted] and TMSC that their success in this particular robbery attempt will happen when pigs flyThank you Ms [redacted] with Revdex.com, [redacted] & [redacted] Enclosures: Copies through

rental wheelchair. Below is our response to the points included the client's complaint: ·         Included in this packet are the order and prescription dated 04/01t2013 that our office received from [redacted] requesting that we provide client with a wheelchair (pgs. 3­5), ·         It is maintained by our office that the client holds the responsibility of ensuring that any provider is in-network with their insurance; however, our records indicate that we were informed by [redacted] that in 2013 this client had a $500.00 annual deductible regardless of provider network status (pgs. 6-7). ·         Also included in this packet are the delivery ticket and patient packet, dated 04/04/2013, that shows that a wheelchair and cushion were delivered to client. The delivery ticket shows that the wheelchair is a rental and the price of $135.79 is documented on the delivery ticket as the rental pricing. Client signed the delivery ticket and patient packet information sheets (pgs. 8-11). ·         Due to the varying insurance policy guidelines, our delivery drivers are instructed to refer clients to the telephone number on the delivery ticket (pg. 8) with any questions regarding billing practices. ·         [redacted] requires that manual wheelchairs be provided on a rental basis for a total of 13 months. [redacted] bases their rental allowable on the [redacted] rental allowable (pg. 25). You can see from the attached Explanations of Benefits that we have only billed the client the amounts allowed by their insurance. (Please note the following: [redacted] will allow 100% of their rental allowable for rental months 1-3 and will only allow 75% of their rental allowable for rental months 4-13). ·         The reimbursement methodology for a [redacted] provider differs from a wholesale online price. Please see the attached [redacted] Supplier Standards (pg. 26) as well as the Customer Rights and Responsibilities (pg. 27) and Provider Responsibilities (pg. 28) that providers must adhere to. The expectations outlined in both of these which are required of [redacted] participating providers have costs that are directly reflected in the allowed amounts set by [redacted]. The client received both the [redacted] Supplier Standards and the Customer Rights and Customer Responsibilities outlines as part of their patient packet.                                         ... Our office maintains that we have handled this account appropriately and to the best of our ability. We understand the client's frustration with the high cost of receiving these services; however, we feel that these high costs are a direct result of [redacted]'s extensive requirements placed on providers. We spoke with both the client and the client's spouse several times throughout 2014 and 2015 explaining our billing procedures and offering them payment plans as well as discount options. Another misstatement, or black lie told by Ms [redacted] is that the prescription for this wheelchair was dated 4/1/2013. You will note that our copy (marked copy #4) of the actual prescription shows 4/3/2013. How could the delivery ticket have been made before the prescription had been written?
