I have reviewed the response made by the business in reference to complaint ID [redacted], and have determined that this proposed action would not resolve my complaint.  For your reference, details of the offer I reviewed appear below.

I am responding to [redacted], owner of Triad Diagnostic Solutions, business response to my complaint. Let us all have the facts:1.       I reported to [redacted] at a business, Triad Diagnostic Solutions.2.       I checked in at the reception desk of Triad Diagnostic Solutions.3.       Triad Diagnostic Solutions receptionist advised [redacted] we had arrived.4.       Triad Diagnostic Solutions receptionist told us [redacted] would be with us momentarily.5.       [redacted] came out of an office of Triad Diagnostic Solutions & asked us to come in.6.       [redacted]’s medical exam was conducted in the room I entered at Triad Diagnostic Solutions.7.       I was struck by [redacted] during the medical exam at Triad Diagnostic Solutions and I was gravely injured by [redacted]’s strike and crippled for life.8.       At no time did Triad Diagnostic Solutions employee, the receptionist, inform me that [redacted] was not employed by Triad Diagnostic Solutions.9.       At no time did [redacted] inform me he was not employed by Triad Diagnostic Solutions.10.      [redacted] was, in fact, held out to me to be employed by Triad Diagnostic Solutions, both by Triad Diagnostic Solutions employed receptionist and by [redacted].11.      Withheld by both Triad Diagnostic Solutions employed receptionist and [redacted] at the date/time of my medical exam, [redacted] now comes with new information that [redacted] is not employed by Triad Diagnostic Solutions, but is a sub-lessee of Triad Diagnostic Solutions.Those are the facts. Given the facts, my contention is clear.1.       [redacted] was held out to me and my partner to be employed by Triad Diagnostic Solutions, which is the causational link for Triad Diagnostic Solutions to be as culpable & liable for the grave, crippling, lifelong injuries I sustained at the hands of [redacted].  2.       If Triad Diagnostic Solutions business policies, practices, & procedures include the choice not to identify or disclose that their sub-lessee, [redacted], although practicing on Triad Diagnostic Solutions premises, is not employed by Triad Diagnostic Solutions, then the affiliation between [redacted] and Triad Diagnostic Solutions becomes one, creating the foundation for Triad Diagnostic Solutions to be as culpable & liable for the grave, crippling, lifelong injuries I sustained at the hands of their sub-lessee, [redacted].3.       Triad Diagnostic Solutions will find it difficult to shelter themselves under this new information that [redacted] was merely Triad Diagnostic Solutions sub-lessee, when [redacted] was distinctly held out to the public as employed by Triad Diagnostic Solutions.4.       [redacted] made a careless, reckless, negligent mistake by striking me and, likewise, Triad Diagnostic Solutions also made a careless, reckless, negligent mistake by holding [redacted] out to be employed by Triad Diagnostic Solutions.4.       It is my right to file a claim against Triad Diagnostic Solutions for injuries I sustained on their premises by an individual held out to be employed by them.5.       It is Triad Diagnostic Solutions responsibility to comply with my request to obtain Triad Diagnostic Solutions liability claims paperwork.6.       I have every expectation that Triad Diagnostic Solutions will forward me, without further delay, their building/property liability insurance claims paperwork and their medical liability insurance claims paperwork, to file my claim. I am not confused, but I hope this clears up some of the confusion [redacted] appears to have. Respectfully,[redacted]

Hi Revdex.com,I'm sorry it took me awhile to respond as I have been out of town. In response to [redacted]'s complaint let us review the facts. [redacted] did come in to Triad's place of business and was greeted by Triad's receptionist and Dr. [redacted] was told by Triad's receptionist that the patient was ready and Dr. [redacted] did ask the patient to come in. Those are definitely facts. But as I stated before, Dr. [redacted] see's [redacted] patients separate of his business with Triad Diagnostic Solutions. Dr. [redacted] is a true Independent Contractor. He works for a number of companies doing reads fro Nerve Conduction Studies (NCV's) and Electromyography (EMG's) which he does do for TRIAD as well. Dr. [redacted] has worked for TRIAD in that capacity since 2006. Dr. [redacted] has also worked for **/[redacted] on the [redacted] reservation for 10+ years as well, independent of his work with TRIAD and he recently started seeing **/[redacted] patients for the state of Arizona in [redacted] where he sub-leases an office from TRIAD. He also worked for [redacted] neurology and [redacted] independently of TRIAD doing NCV and EMG's. As an Independent contractor, Dr. [redacted] carries his own separate liability/malpractice insurance as an independent contracting Dr. is required to do. TRIAD receives no compensation from the state of Arizona for the [redacted] patients' he sees in the room he sub-leases from TRIAD. TRIAD's receptionist does not receive any compensation from Dr. [redacted] for checking Dr. [redacted]'s patients in and escorting them to his sub-leased office. TRIAD does this as a courtesy for Dr. [redacted]. I am truly sorry for what happened to Mrs. [redacted] but this is an issue she has to take up with Dr. [redacted] and not TRIAD. evaluations every other Tuesday and some Monday's. This patient that filed a complaint is not a patient of Triad at all and has nothing to do with our business. Triad does diagnostic testing for nerve damage and does not do any work at all with [redacted] and [redacted]. I think the patient got confused because Dr. [redacted] is in Triad Diagnostic Solutions' office which is where the confusion probably came from. As far as the certified letter goes, Triad was and never has not signed for a certified letter. We get them all the time and sign for them. Sometimes there is a sticker on our office door if no one is here and the door is locked like during lunch hours which is maybe why that letter was not signed for. Also, we would have not received a certified letter from [redacted] for that patient because Dr. [redacted]'s mailing address for [redacted] is his home office, not Triad's. I hope this clears up some of the confusion as Triad has been part of the Revdex.com for 4 years now and has never received one complaint. Review: I have attempted to contact Triad to obtain their liability insurance papers so I can file a claim. They refused receipt of my contact letter mailed by me on April 30, 2015 via [redacted] return receipt requested.

I want their liability insurance papers so I can file my claim.

I am a 100% [redacted] (since 2011 ... was 40% s/c before that). Also since 2011, I began receiving [redacted] (100%). BUT ...

On February 2, 2015 ... I was sent to have a routine medical exam with Triad's doctor [redacted], AZ Medical Lic#[redacted]. AFTER 15-20 minutes of discussion with [redacted] about my 39 year old ACTIVE painful muscle spasm ... [redacted] STRUCK ME on IT - without even lifting the long-sleeve of my sweater jacket to SEE IT or see WHAT he was striking!! The trauma from his strike IMMEDIATELY sent my whole body into the incurable disease of RSD - which is "the wasting away" of all the muscles in my body ! The [redacted] from [redacted]'s strike was IMMEDIATE and continual agony - that day, that night, and every single solitary day since.

That was Feb. 2nd. On Feb 10th & still in excruciating pain, I was seen by my family doctor who IMMEDIATELY said my spasming muscles were SEEN !

On Feb 24th, I was seen by the 1st neurologist who diagnosed my RSD & specifically stated it was caused from the trauma of [redacted] striking my active 39 year old muscle spasm [redacted].

That was Feb 24th ... On March 5th ... I could no longer walk & the ** issued me a wheelchair ... from that point, I've been seen by TWO [redacted] management specialty doctors who both said they've "never seen anything this bad before" & "there's nothing they could do for me". And I've been seen by my family doctor again - my ** doctor twice - have had numerous tests (including xrays & mri's), I'm on narcotics for the horrific [redacted] & constant muscle spasms & deterioration ... AND I'm now seeing yet ANOTHER neurologist & scheduling is in progress for a bone scan & a rheumatologist specialty doctor.

In just over 3 months, my whole body is afflicted with the most horrific [redacted] any human could endure ... with every bit of my skin is so sensitive that even a fold in my clothing hurts me horrendously. Inconsolable sobbing, from the maddening unrelenting pain, is a regular occurrence. (I never even knew or fathomed it was possible for a human to have THIS much [redacted] without dying ...)

In just over 3 months, I went from being disabled solely on my right upper body, arm & neck ... to being a cripple. My husband hasn't been able to work since Feb 2nd ... he's been taking care of me (...and that includes dressing me). The only thing that works is my fingers - and my right hand fingers are fading daily, so I never know if I'll even be able to use my computer from one day to the next. Some days - I can't ... those days I just sit, sobbing uncontrollably ... in the most horrifying [redacted] imaginable. Even the movement of the car taking me to doctor appointments sets EVERY [redacted] active - my arms, my legs, my neck - my whole body. By the time I get to an appointment, I'm sobbing - again - in horrific pain. I'm living a nightmare - so is my husband. And to make matters even worse ... THAT doctor ([redacted]) told [redacted] I was "better" and they stopped my [redacted] payments & now I have to fight to get that back ... cost me $3500 just to hire a [redacted] advocate to represent me ... and THAT doesn't include nearly $2000.00 out-of-pocket expenses I've already paid for doctors, medications, etc. !

Triad has refused the certified letter I sent them, requesting their medical liability insurance paperwork so I could file a claim. [redacted] has refused his certified letter, as well.

My supporting documentation is complete, substantiating this claim.Desired Settlement: I want their liability insurance paperwork, so I can file my claim for being injured at their place of business.
