I purchase [redacted] through [redacted]'s special pharmacy. I paid it out of pocket. A $10 rebate form came with the medication. I filled out the form, attached the required original receipt, and mailed it to the above address according to the instruction. In the instruction, it doesn't say what information is needed on the receipt. All it says is to provide the original receipt. One month later, I received a mail saying I am not eligible for the rebate because I didn't provide the original receipt that shows product name, quantity, NDC, date purchased, name, amount paid. But the receipt I provided is the only receipt I got from the pharmacy which shows the medication #, quantity, date purchased, amount paid. When my healthcare provider prescribed [redacted] I was given a $10-off coupon that 'the pharmacy might accept, but if they don't, it can be mailed in'. Since my pharmacy did not honor the coupon, I followed the instructions on the coupon exactly: "If your pharmacy is unable to process this coupon, please complete the back of this coupon and mail it, along with your pharmacy receipt to..." A few weeks later I received a notice that my rebate had been denied due to additional restrictions: "...an original pharmacy receipt that shows the pharmacy name, patient name on prescription, name of medication purchased, price paid and date purchased." It took two (inconvenient) trips to the pharmacy in order for me to secure a receipt that matches the requirements were added on AFTER I had complied with the original instructions. I believe that it is fraudulent to add on restrictions AFTER the original requirements are met. It seems clear to me that the rebate program is designed to encourage consumers to purchase [redacted] (expecting a $10 rebate), but also designed to incur such a burden of cost (in time and money) that many/most purchasers will give up, deciding that the rebate is not worth it.Product_Or_Service: [redacted]Account_Number: XXXXXXXXXXX/XXXXXDesired SettlementI have submitted the second receipt, which complies with the restrictions that were added AFTER my original submission. I hope that, at the very least, I will receive my $10 rebate. In addition, I feel that the [redacted] Savings Program should be somehow penalized for fraudulent activity.Business Response The customer did not submit all necessary items as stipulated by the instructions in the offer. Upon receipt of their submission, we sent a rejection letter that stipulated the customer did not include a pharmacy receipt that displays the name of the medication purchased. If the customer decides to re-submit that claim with all necessary materials, it will be processed accordingly and they will receive their rebate. rebate for [redacted] limit of 8 when savings card specifically states 12 rebates.To whom it may concern: I have been using [redacted] mg twice a day since September 2013 with good results.I am having a very unsatisfactory experience with your savings card rebate program.The savings card I printed in January of this year states it is good for up to 12 uses per year at the same dose.I am being told the rules have changed and it is good for only 8 uses per year.Starting January 2015 it will be good for 12 uses. This leaves me on the hook for $400.I can't afford this already overpriced medication and will have to have my doctor change medications.This is no way to run a rebate program. Perhaps I should contact the Revdex.com or my state attorney generals office.Please let me know if there is anything that can be done about this.
