We received the dispute you sent us for [redacted] assigned ID [redacted] I emailed the patient requesting any additional information since patient was not on our databasePatient replied back and attached billing statements that came from DrTranI replied informing patient DrTran does not work out of our practice and provided her with his mailing address ( [redacted] Fountain Valley, CA )Please let me know if I have resolved this from our end [redacted] [redacted]

Re: [redacted] [redacted]To Whom It May Concern,The consumer was seen in the office. We billed her insurance $360 for the office visit and $430 for a CAM BOOT walker. Her insurance has delay payment PENDING receipt of additional information requested from the consumer-An Le. In other word, the insurance company was waiting for her to reply to their request for additional information in order to process and paid for the medical bill. However, she has notprovided the request information.Attached are the dated notes from our 3" party billing company. As you can see, they have been calling her numerous time and telling her to respond to her insurancecompany request. The insurance company has sent her 3 letters. Her refusal to respond to her insurance company cause the claim to be denied. As such, we arebilling her.As you can see from her attached response letters, she is not cooperating to resolve this issue.Sincerely,

It is not acceptable that the office quoted me $10.00 copayment, and when they failed to bill insurance, they turn around and bill me. I accept the service in good faith that it only costs me $10.00 without any disclosure that if the office failed to bill insurance, I will be responsible for the billing. The office provided me falsified quote and I refuse to be held responsible afterward.
