In response to [redacted] , I will restate that Trinity has a grandfathered plan that meets all requirements of grandfathered plans with the ACA Trinity provides an employee handbook and benefit summary on their internal website that gives all the information regarding this ALL employees are told it is their responsibility to become familiar with those documents at the time of hiring Also, in response to [redacted] stating had she would have known this she would have gotten a plan through the marketplace, the marketplace would not have been an option as she is employed by a facility who offers health insurance benefitsAgain the services/charges are correct, and the balance remains patient responsibility

I am rejecting this response because: I have been in contact with [redacted] and while they are working with my insurance to resolve this issue, I received another bill for $Until these outstanding bills are resolved I will not accept or resolve this issue

We spoke with the patient's spouse on 1/2/and explained that the refund had been approved and would be sent out on 1/10/ The refund has been sent and the patient should have received this in the mail If the patient has not received the refund they can feel free to contact me directly at ###-###-####Thank you

I am rejecting this response because: I was never made aware when I signed up for this healthcare policy, that it did not adhere to Federal Laws, namely ACAI was made to believe that this health policy, just like any other sold after 2010, included the new regulationsNot at one point has anyone ever informed me that this policy is outdated and has never been updated to comply with federal law, let alone that this is illegal - there was a time frame of years given to health care plans to adjust to the new federal laws, and I signed up n and the policy should have reflected ACAIf it truly was an outdated policy, it does not comply with federal laws and therefore should not be allowed to be sold, especially not under the disguise of it being an "up to date policy"I will not be paying this amount, since I was not made aware that this policy is non compliant with federal law, it also does not state anywhere in the policy booklet I received from Trinity HealthThis is a shamI would have never signed up for this policy if I knew it was non compliant, I would have signed up for a policy through market place insteadI will inform the Attorney State General about this

We spoke with the patient on the same day the complaint was made and explained the Bad Debt payments and the other payments she had made
on the account. The Bad Debt was paid in full and there is no refund due to the patient. We also worked with her on other payments in which she had questions on and she was okay with this. There was one date of service that required follon our end and we are working with the insurance company on that charge. Also, I called and spoke with the patient on Saturday 8/27/and she didn't have any follquestions and as my direct number for any questions

The patient's insurance company processed and paid the account on 5/1/15. Trinity sent the employee the first statement on 6/15/15. This was within 30 days of the processing by the insurance company. In order to receive the discount the...

patient/employee would have needed to pay the amount due after discount by 7/1/15. The employee did not make a payment until 7/22/15 which is more than two weeks after the due date. Trinity called the employee on 7/22/15 and he asked that he be called back as he was busy at the moment. The policy regarding Employee Discounts at Trinity which is available on Trinity's intranet and handbook states that the discount can be applied up to 60 days from the EOB date from the insurance company, which the patient receives a copy of from the insurance company. The balance due on the account is patient responsibility as they did not follow the guidelines of the policy.

For most of the same reasons that are in my complaint, plus they did not send me a bill for 60 days of the ins. co. pmt., not 30 like they are stating,( read their statement), and they gave me less than 2 weeks to pay the bill. Also I cannot always be bothered while I am at work, and they did not call me back again. I was not even aware of the 60 day policy either, however I received the bill after that time limit.

Trinity's policy on employee discounts has not changed. The policy has been 60 days from the date the insurance company processes the payment. I apologize that the patient missed the deadline for receiving the employee discount. In the previous response the information taken directly from the Trininet states that the policy is 60 days from the EOB date. Once again my apologies to Mr. [redacted] but the balance due on the account remains patient responsibility.

have collection notices stating there are 2 seperate $40 amoutns sent to a collection agency n Minot, ND. These amounts were paid to Trinity for Specialist services for my Son in 2o12 by the Boys Scouts of America an in a previous complaint reply they stated they would be recalling all billing for [redacted] invlolving his broken leg from all collection agencies and submittig it through the proper insurance companies. I have also been paying steadily through 2 different bank accounts on automatic payments for 4 different billing accounts with trinity and some of these also appear to have been mis applied and turned over to collections. the practices of the billing office of this hospital and doctors group is criminal. I want full accounting of charges, insurance payments, Insurance deductions for being in-network, co-payments, Personal payments and records from the collection agencies since 2010 for [redacted] & [redacted]. I have a letter from the previous complain apologizing for the issues and saying they would be corrected. They have not been corrected.

We have verified with the collection agency in Minot ND that the 2 separate $40 accounts are not for [redacted]'s charges. The accts for [redacted]'s charges related to the Boy Scout of America were closed and returned by the agency when the customers first complaint was logged in 2013 with Revdex.com. The collection agency at that time asked that those amounts for [redacted]s Boy Scout related charges be removed from the customers credit report. The customer will need to contact the collection agency to find out more information on the accounts currently with the agency. The agency has not sent any notices on the accounts we had returned for [redacted]. We will provide the customer proof from the Collection agency via mail.
We will provide another itemized for all accounts and will send the documentation and itemized via certified mail.
We cannot address [redacted]'s husband in this response, he would have to contact our office directly or sign a release of information for us to disclose to [redacted].
As we have addressed prior [redacted]'s Boy Scout related charges were correct and rebilled in response to the 2013 Revdex.com Complaint to which the customer did not respond at that time. Case #[redacted].We did receive payment from Boys Scouts of America for those charges in Oct 2013. All the detail was included in our written response to the customer in 2013.

This is a sham. I would have never signed up for this policy if I knew it was non compliant, I would have signed up for a policy through market place instead. Hello,The patient has a policy with Trinity Health and this policy is a fully self-funded grandfathered policy.  As such, there are parts of the ACA that do not apply to fully self-funded grandfathered policies.  Also, the coding was reviewed and deemed correct, this was not done as a...

preventative measure.  Blue Cross did process the claim and left the balance to patient responsibility.  Therefore the account is correct and [redacted] still owes the remaining balance of $475.41 at this time.
