Ms [redacted] presented to our office on Jan25th, as a returning patientShe initially stated shewas interested in esthetic work on her front teethUpon full examination, we discussed options ofrestoring her front teeth to improve her appearanceI also diagnosed problems on ***'s posteriorteeth as wellShe has large, worn silver fillings that have recurrent decay as well as stress fracturesWemade [redacted] aware of these findings and discussed pros and cons of restoring the worn posterior teethor doing the cosmetic work firstAt this appointment we took impressions of her teeth to bring herback for consult appointment and to prioritize her treatment On January 11, [redacted] returned for her consult appointment to see the proposal for her teeth anddecide where to startShe stated that since the crowns for the front are cosmetic she preferred to starton the crowns on the posterior teeth to prevent further break down to ensure she does not have todeal with any emergencies or broken teeth down the roadAt this appointment we reviewed intra oralpictures of her teeth, revisited the treatment plan again and advised [redacted] to take plaster casts hometo show her husband I asked her to call me if she had any concerns or questions On January 26, 2016, [redacted] came in for treatment which was consisted of filling and a crown on thelower right sideUpon removal of the old fillings and recurrent decay, a deep crack wad detectedItook intra oral photographs for [redacted] to seeWe discussed the possibility of future root canaltreatment for that tooth due to the amount or recurrent decay and fracture causing by old silver fillings.At this appointment I filled and prepared the tooth for a crown and it was temporized [redacted] calledonce for bite adjustmentWhen she came in to seat her crown , we decided to cement her crown withtemporary cement in case the nerve didn't settle down and a root canal was needed On February 16, 2016, [redacted] came in and reported that tooth was feeling much better and she wasready to recement the crown with permanent cementAt every appointment [redacted] was remindedabout the fracture in the tooth and we discussed cracked tooth syndrome in detail with possible needfor future root canal treatment [redacted] came in one more time after crown was seated permanently fora bite adjustment and after that I advised her to call if more adjustment was needed [redacted] cancelled her future appointments and she did not respond to our callsWhen she was sent aletter regarding her past due account she called back upset saying that she went to another dentist whotold her she might need root canal and that the crown was made out of material that is too hard[redacted] may need a root canal for the reasons I've previously stated, but the material used in approvedand appropriate for posterior teethPerhaps the other dentist she saw has some ulterior motive forcriticizing my treatment At this point we decided to write off her balance as good will and my insurance coordinator called her tolet her knowI called [redacted] and left a message for her to call to discuss and clarify anymisunderstandingsIf you have any other questions regarding my treatment for ***, please contactme Sincerely,Pari S***, DMD

DrS*** did the right thing by crediting my account of the and I am satisfied with that, but her statement that she advised me of the root canal is notI was never called by her office for any reason and they failed to bill my insurance for the balance of due to staffing turnover issues within their officeHer Office Manager told me that directly when I had to call her to find out why my insurance was not billed in the month in which the crown was doneThis is totally on them and shame on them for trying to blame me
