I scheduled an "annual exam"This "reason for visit" is clearly documented as "1.annual visit"However their diagnosis codes are listed "anxiety-primary" it is no listed until the 4th code as "encounter for preventative health examination"This "preventative health" code should be listed first as the code as the primary reason for visit is "annual exam"For ears JHH and my insurance have agreed that these lines should match up but Hopkins will not make the changesToday JHH sent me a reason for billing the way they did was "to establish care with complaints of fatigue and rash,"This is totally inconsistent with any other reason they've given me in yearsNot to mention if they are now saying this was my reason for the visit, why did they code the visit as "anxiety."
