*** *** was seen by our Katy locationWe didn't collect anything from him at the time of service because we were not for sure what his out of pocket would be at that timeSince he is a *** he is allowed one exam without a referral with a participating ophthalmologist but that visit
would be applied towards his deductibleWe have contacted *** and they informed us that the claim was processed according to his policyWe are happy to discuss this with *** *** directly and work with him on any concerns that he might have regarding his policy
