September 2, 2016Dear Revdex.com Representative:We received a fax on 8/15/from Mr [redacted] directing us to cancel policy number [redacted] effective on 8/12/Please note that we cannot back date a cancellation request, and our system won’t allow us to do thatWe can only cancel as of the current date or later than the current dateThe last payment he made was processed on 8/06/in the amount of $149.91, and the policy was paid only to 8/29/2016.In order to cancel a policy, we require a written request along with the following information:Policy number:Reason for cancellation:Effective date: (has to be today's date or in the future)Printed name:Signature:Please note that we received the fax on 8/15/to cancel policy number [redacted] effective 8/12/without any signatureWe could not back date the cancellation to 8/12/as he requested, because the request was sent and received on 8/15/The premium for any policy is pro-rated, so we’re talking about only days of unused premiumTherefore, the policy was short-rated, which wouldn’t have come nearly close to $149.91.I called the client the same day his fax was receivedThere was no answer, so I left a message explaining why we couldn’t back date policy cancellation requests, and gave him the information he needed to provide in the cancellation requestI also sent the client an email with this same informationI did not hear back from him until 8/29/(days later)I tried to explain the reason we couldn’t process his cancellation request, but that was not satisfactory to himI have included the letter that was faxed to us on 8/15/We regret any inconvenience this has caused our client, but we did our best effort to resolve this simple issue.Thank you,Regards, [redacted] office [redacted] fax [redacted] *** https://ultracarinsurance.com

The premium for any policy is pro-rated, so we’re talking about only 14 days of unused premium. Therefore, the policy was short-rated, which wouldn’t have come nearly close to $149.91.I called the client the same day his fax was received. There was no answer, so I left a message explaining why we couldn’t back date policy cancellation requests, and gave him the information he needed to provide in the cancellation request. I also sent the client an email with this same information. I did not hear back from him until 8/29/2016 (14 days later). I tried to explain the reason we couldn’t process his cancellation request, but that was not satisfactory to him. I have included the letter that was faxed to us on 8/15/2016.
